Objective Through univariate and multivariate analysis to find out the risk factors of Low cardiac output syndrome(LCOS)after coronary artery bypass grafting(CABG)for coronary heart disease(CHD),with a view to preoperatively for patients who are planning to undergo coronary artery bypass grafting The evaluation provides a basis and provides clinical guidance for preventive interventions during and after the operation.Methods Selected 452 patients who underwent coronary artery bypass graft surgery for coronary atherosclerotic cardiomyopathy in the Department of Cardiac and Great Vascular Surgery of×××Hospital from January 2015 to December 2019 as the research objects,and retrospective analysis was performed.According to whether low cardiac output syndrome occurred after operation,they were divided into experimental group and control group.Use the hospital office system to collect various indicators and data of the two groups of subjects:(1)Preoperative clinical data:age,gender,history of hypertension,history of diabetes,history of preoperative myocardial infarction,history of preoperative PCI,cardiac function(left ventricular injection)LVEF%),preoperative hemoglobin value,preoperative creatinine value,preoperative valvular disease;(2)Intraoperative data:surgical method,operation time,left main disease,number of diseased vessels(number of grafts),intraoperative blood loss,Aortic block time,cardiopulmonary bypass time;(3)Postoperative indicators:arrhythmia,postoperative hemoglobin value.Single factor and binary logistic regression analysis were performed in turn to determine the independent risk factors for low cardiac output syndrome after coronary artery bypass grafting.Results(1)In the study of 452 CHD patients,55 of them developed LCOS after surgery,the incidence rate was 12.17%.(2)The analysis showed that the preoperative age>60 years old,diabetes history,preoperative myocardial infarction history,PCI history,cardiac function(left ventricular ejection fraction LVEF%<45%),preoperative arrhythmia,preoperative hemoglobin value,Significant differences in left main disease,operation time,number of grafts,intraoperative blood loss,postoperative arrhythmia history,and postoperative hemoglobin value were significantly different(3)Logistic regression analysis showed that the left main disease,intraoperative blood loss,postoperative arrhythmia history and postoperative The occurrence of LCOS was significantly correlated(p<0.05).Left main disease,intraoperative blood loss,and history of postoperative arrhythmia are independent risk factors for postoperative LCOS.Conclusion Age> 60 years old,history of diabetes,history of preoperative myocardial infarction,history of PCI,cardiac function(LVEF<45%),preoperative arrhythmia,preoperative hemoglobin value,operation time,number of grafts,and postoperative hemoglobin value are all CHD Independent influencing factors for LCOS after CABG in patients;left main disease,intraoperative blood loss,and history of postoperative arrhythmia are independent risk factors for LCOS after CABG in CHD patients. |
