| Objective:To compare the analgesic effects of butorphanol and dezocine in postoperative patients with mechanical ventilation in ICU(Intensive Care Unit).Methods:This study was a prospective randomized controlled study.From August 2020 to December 2020,the Department of ICU of the fourth Hospital of Hebei Medical University was selected as the critical patients who needed mechanical ventilation during the perioperative period.They were randomly divided into butorphanol group and dezocine group.The patient’s Numerical Rating Scale,Critical Care Pain Observation Tool,heart rate,mean arterial pressure,respiratory rate were observed and recorded before the start of infusion of the study drug,and every 20 minutes for the first 6 hours,then evaluated every hour until the shut down.To compare the analgesic and sedative effects,haemodynamics effects,medication differences and safety indexes in the two groups.Results:There was no significant difference in basic medical characteristics and critical illness score,that is,Acute Physiology and Chronic Health avaluation Ⅱscore,which indicated that the two groups were comparable.The results of this study showed that the percentage of time of analgesia in all of the observed time points in the butorphanol group was significantly lower than that in the dezocine group,(84% vs.90%,P<0.05).The dosage of propofol in butorphanol group was lower(713.04±373.93 mg vs.966.00±625.52 mg,P < 0.05),There were 1(2%)patient in the butorphanol group who received midazolam,9(18%)patients in the dezocine group,and 8(17%)patients in the butorphanol group who received remifentanil,and 2(4%)patients in the dezocine group,with statistically significant differences(P<0.05).There was no significant difference in mechanical ventilation time between the two groups(26.43±15.93 h vs.25.76±16.97 h,P > 0.05).Compared with dezocine group,the ICU hospitalization time in butorphanol group was prolonged(77.67±36.76 h vs.64.60±26.95 h,P<0.05).The cost of analgesic and sedative drugs in butorphanol group was significantly lower than that in dezocine group(1914.09±2464.642 yuan vs.2967.66±2581.20 yuan,P < 0.05).The difference was statistically significant.Between the two groups,there was no statistical difference in the cost of mechanical ventilation(10806.09±7209.51 yuan vs.10298.14±6517.18yuan),ICU hospitalization(25746.04±14156.72 yuan vs.21280.20±7857.16yuan)and total hospitalization(95464.02±29500.09 yuan vs.97405.56±29326.93yuan).In addition,There was no significant difference in the incidence of adverse reactions between the two groups.Conclusions:1.Both butorphanol and dezocine had good analgesic effect in ICU patients with mechanical ventilation after operation,and dezocine had better analgesic effect than butorphanol.2.Butorphanol has stronger sedative effect than dezocine and requires less propofol and midazolam to maintain sedation target.3.The cost of analgesic and sedative drugs during mechanical ventilation was lower than that in dezocine group in butorphanol group,and it had good clinical economic applicability.4.The adverse reactions of butorphanol and dezocine are less and the clinical application is safe,so it is worth popularizing and applying in clinic. |
