Objective: 1.Analyze the relationship between Betatrophin,C reactive protein(CRP)and polycystic ovary syndrome(PCOS)insulin resistance(IR);2.Research and explore whether Betatrophin and C reactive protein can be used As an indicator of the efficacy of the treatment of PCOS-IR.Methods: In the cross-sectional study,we collected 200 PCOS patients as the study group and 60 healthy women with regular menstrual cycle IVF physical examination as the control group.According to the homeostatic model assessment insulin resistance(HOMA-IR),the experimental group was divided into IR group and non-IR group.On the 2-3 days of the menstrual cycle or when no mature follicles were found in the color Doppler ultrasound examination,fasting venous blood was drawn from all subjects in the morning.A specimen was tested in the laboratory of our hospital for glucose metabolism-related indicators,sex hormones,and androstenedione(androstenedione,AND).Another specimen was centrifuged at 3000 rpm for 15 minutes in the reproductive center of our hospital and the supernatant was extracted,and the serum was quickly divided into the refrigerator at-80℃ for Betatrophin,Determination of C-reactive protein levels.All subjects collected general information(including age,height,weight,menstrual cycle,hairiness,acne,age at menarche,years of infertility).Fasting blood glucose was measured by hexokinase method;fasting insulin,sex hormones and AND were measured by E602 Roche electrochemiluminescence method;enzyme-linked immunosorbent assay(ELISA)was used to measure serum Betatrophin and CRP levels.At the same time,we performed the combined treatment of metformin and Diane-35 on 70 PCOS patients,and repeated the measurement of related indicators 2 months later.Using statistical methods,analyze the relationship between Betatrophin and CRP levels and IR in PCOS patients,and explore whether drug treatment has an effect on Betatrophin and CRP.Results:(1)Serum related indicators: The levels of LH,T,AND,INS,HOMA-IR,CRP,and Betatrophin in the experimental group were significantly higher than those in the control group,with a significant difference <0.05);and the above indicators in addition to CRP,the IR and non-IR groups were also compared with the control The difference between the two groups was significant(p<0.05),all higher than the control group;CRP levels were significantly different among the three groups,IR group>control group>non-IR group(p<0.05);IR group LH,T,AND,GLU,The levels of INS,HOMA-IR,CRP,and Betatrophin were significantly higher than those of the non-IR group,with statistical differences(p<0.05);FSH,PRL,E,and P were not significantly different among the three groups(P>0.05).(2)General clinical data:(1)The BMI level of the experimental group was higher than that of the control group,with statistical difference,and the BMI level was significantly different in the IR group>non-IR group>control group(P<0.05);(2)The experimental group had 142 menstrual irregularities Cases,accounting for 71.00%(142/200),99 cases in the IR group,accounting for 76.74%(99/129),43 cases in the non-IR group,accounting for 60.56%(43/71),menstruation in the control group Irregularities were 0 cases,accounting for 00.00%(0/60),and there was a statistical difference between the last three groups(76.74%>60.56%>0%)(P<0.05);(3)The experimental group was accompanied by acne in 66 cases,Accounting for33.00%(66/200),which is significantly different from the control group without acne(P<0.05),and the non-IR group in the experimental group is still meaningful compared with the control group(P < 0.05).The number of cases with acne in the IR group accounted for37.21% and the non-IR group accounted for 25.35%,the difference was not significant(P>0.05),meaningless;(4)The experimental group with hirsutism was 78 cases,accounting for 39.00%(39/200),the control group has 0 cases with hirsutism,accounting for 00.00%(0/60),in the experimental group,the non-IR group has 24 cases with hirsutism,accounting for 33.80%(24/71),IR The proportion of 41.86% of the group was higher than the proportion of 33.80% of the non-IR group,but the two were statistically insignificant(P greater than0.05),while 39.00% of the experimental group and 33.80% of the non-IR group were compared with the control group.The statistical difference was significant(P<0.05);(5)Age,menarche,and infertility years were not significant in the comparison between the three groups(P>0.05).(3)In the correlation analysis,HOMA-IR shows an upward trend with the increase of T,LH,AND,INS,CRP,and BMI,and there is a significant positive correlation.And CRP is significantly positively correlated with LH,AND,INS,Betatrophin,and BMI;(4)Linear stepwise regression analysis of HOMA-IR influencing factors suggests the irregularity of AND,T,PRL,and these indicators are significantly positively correlated with HOMA-IR;(5)Linear stepwise regression analysis of Betatrophin’s influencing factors suggests that CRP are significantly positively correlated with Betatrophin,and BMI is significantly negatively correlated with it;(6)Linear stepwise regression analysis of the influencing factors of CRP showed that AND,Betatrophin,INS,PRL,BMI were significantly positively correlated with CRP,and the number of years of infertility was significantly negatively correlated with it;(7)Binary logistic stepwise regression analysis of the influencing factors of PCOS shows that both LH and Betatrophin are risk factors for PCOS;(8)After 8weeks of treatment with formin combined with Diane-35,70 women with PCOS repeated the measurement of related indicators.The study found that BMI,LH,AND,INS,T,HOMA-IR,CRP,Betatrophin after treatment all decreased compared with before treatment,with statistical differences(P<0.05).Conclusion:1.PCOS patients have higher levels of LH,T,AND,and are characterized by hirsutism,acne,irregular menstruation,and insulin resistance;2.The increase in CRP levels in PCOS patients suggests that its occurrence is related to chronic subclinical inflammation,and there is a positive correlation between CRP and IR;3.Betatrophin levels in PCOS patients are elevated,and no correlation is seen in other IRs;4.CRP levels and Betatrophin levels in PCOS patients are positively correlated and affect each other;5.After 2 months of treatment with the insulin sensitizer metformin combined with Diane-35,the levels of Betatrophin and CRP decreased,and the degree of insulin resistance was improved.Therefore,the levels of Betatrophin and CRP can be used as indicators of the therapeutic effect of PCOS-IR;6.Both LH and Betatrophin are risk factors for PCOS.
