Clinical Applied Study On Improving The Biopsy Diagnosis Rate Of Thyroid Nodules Cytological Fine Needle Aspiration Guided By Multi-modality Ultrasound

ObjectiveTo explore the value of multi-modality ultrasound guided fine needle aspiration(FNA)cytology biopsy of thyroid nodules in order to improve the diagnosis rate of the biopsy.MethodsFrom January 2018 to January 2020,338 patients with 356 nodules underwent FNA of thyroid nodules were enrolled into this prospective study.They were assigned into four groups randomly,including: conventional ultrasound-guided FNA(US-FNA)group,contrast-enhanced ultrasound-guided FNA(CEUS-FNA)group,ultrasound elastography-guided FNA(UE-FNA)group,and the multi-modality-FNA group(combined with US,CEUS and UE).The FNA biopsy cytology result of each patient was analyzed and compared the diagnosis rate of biopsy in different groups.ResultsThe biopsy diagnosis rates of multi-modality-FNA,UE-FNA,CEUS-FNA and US-FNA groups were 89.9%,73.9%,71.6% and 54.9%,respectively.The difference among these four groups was statistically significant(p<0.001).Pairwise comparison between the groups after corrected the inspection standard(α=0.008),p<0.008 was statistically significant for multiple comparisons between groups.The biopsy diagnosis rate of multi-modality-FNA group was higher than UE-FNA,CEUS-FNA and US-FNA groups(89.9% versus 73.9%,p=0.006;89.9% versus 71.6%,p=0.002;89.9% versus 54.9%,p<0.001).There was no statistical difference between UE-FNA,CEUS-FNA and US-FNA groups(73.9% versus 71.6%,p=0.735;73.9%versus 54.9%,p=0.008;71.6% versus 54.9%,p=0.021)。ConclusionMulti-modality ultrasound guided FNA of thyroid nodules was helpful to select the malignant thyroid nodules and locate the malignant target areas in thyroid nodules more accurately.Therefore,the diagnosis rate of biopsy in FNA was great improved which has important clinical application value.
