Objective: To investigate and study the clinical effect of the modified Buyang Huanwu Decoction treatment in diabetic peripheral neuropathy(DPN),comparing the changes of serum insulin-like growth factor(IGF-1)level before and after the treatment,so as to explore the influence and mechanism of IGF-1 level in the modified Buyang Huanwu Decoction treatment after DPN.Method: From April 2019 to January 2020,60 patients with DPN of qi deficiency and blood stasis type were randomly divided into the treatment group and the control group,with 30 cases each group.The control group was given Mecobalamin orally,and the treatment group was given Mecobalamin and modified Buyang Huanwu Decoction orally for 12 weeks.Before and after treatment,TCM syndrome score,TCM symptom scores,Michigan diabetic peripheral neuropathy(MDNS)score and serum IGF-1 level were measured,and the correlation between serum IGF-1 level and MDNS score were compared after treatment.T test was used for measurement data,variance test for counting data,Pearson correlation analysis was used for correlation analysis,and Ridit analysis was used for grade data.Results:1、After treatment,the TCM syndrome scores of the two groups were significantly reduced(P < 0.05);the improvement of TCM syndrome scores of the treatment group was significantly better than the control group,the difference was statistically significant(P < 0.05).2、After treatment,the two groups of patients with limb numbness,pain,fatigue,shortness of breath,sluggish speech and other TCM symptom scores decreased(P < 0.05);the enhancement of TCM symptoms in the treatment group was better than the control group,and the difference was statistically significant(P < 0.05).3、After treatment,the therapeutic effect of the treatment group was increased than the control group(P < 0.05),the difference was statistically significant(P < 0.05).4、After treatment,the MDNS scores of the two groups were all decreased(P <0.05),the changes of MDNS scores of the treatment group was significantly better than the control group,the difference was statistically significant(P < 0.05).5、After treatment,the serum IGF-1 level of the two groups was increased(P <0.05),and the serum IGF-1 level in the treatment group was significantly higher than the control group(P < 0.05),the difference was statistically significant(P < 0.05).6、After treatment,the correlation between serum IGF-1 level and MDNS score showed that IGF-1 level was negatively correlated with MDNS score.Conclusion:1、Modified Buyang Huanwu Decoction can improve TCM Syndromes and increase the TCM syndrome efficacy of DPN patients with Qi deficiency and blood stasis type.2、Modified Buyang Huanwu Decoction can improve the TCM symptoms of limb numbness,pain,fatigue,shortness of breath,sluggish speech,night sweat in DPN patients with Qi deficiency and blood stasis type.3、Modified Buyang Huanwu Decoction can reduce MDNS score of DPN patients with Qi deficiency and blood stasis type.4、Modified Buyang Huanwu Decoction can improve the IGF-1 level in DPN patients with Qi deficiency and blood stasis type,and the IGF-1 level is negatively correlated with MDNS score.It is considered that modified Buyang Huanwu decoction may have therapeutic effect on DPN patients with Qi deficiency and blood stasis type by enhancing the expression of IGF-1. |
