Glomerular Filtration Dysfunction Is Associated With Cardiac Adverse Remodeling In Menopausal Diabetic Women

Objective:To investigate the relationship between impaired glomerular filtration function and cardiac remodeling in menopausal women with type 2 diabetes mellitus.One thousand two hundred and thirty-one hospitalized menopausal females with type 2 diabetes were collected in our hospital.Methods:The age,body mass index,fasting blood glucose and echocardiographic parameters were measured.Estimated glomerular filtration rate(e GFR,ml/min·1.73m~2)was calculated by formulas.e GFR≥90 was defined as normal group(n=558);60≤e GFR<90 as mild diabetic nephropathy group(n=415);30≤e GFR<60 as moderate group(n=182);e GFR<30 as severe group(n=76).Results:Correlation analysis showed that e GFR was negatively correlated with age,fasting blood glucose,hypertension,systolic blood pressure,diastolic blood pressure,pulse pressure,right atrial diameter,right ventricular diameter,left atrial diameter,interventricular septal thickness,left ventricular posterior wall thickness,left ventricular end systolic diameter and left ventricular end diastolic diameter(P<0.05),and positively correlated with body mass index,high density lipoprotein cholesterol and left ventricular ejection fraction(P<0.05).Regression analysis showed that moderate severe e GFR reduction was independently correlated with enlarged left-sided atrial and ventricular chamber,increased ventricular wall thickness,decreasd ejection fraction and secondarily dilated right ventricle.(P<0.05).Conclusion:Impaired glomerular filtration function is associated with left-sided cardiac remodeling and dysfunction secondarily right ventricular dilation in menopausal women with type 2 diabetes.
