| Objective: to analyze the effect of Bushen Zhuanggu Decoction on reducing the risk of adjacent vertebral refracture after percutaneous vertebroplasty(PVP).Through the study,we observed a traditional Chinese medicine prescription to reduce the fracture after PVP.Methods: The vertebral compression patients of fracture in our center Hospital of Nanchang Hongdu of Traditional Chinese Medicine received PVP at the vertebral compression fracture center of osteoporotic center between December 2019 and December 2020.According to the diagnosis and inclusion criteria of traditional Chinese and western medicine,a total of 90 patients meeting the requirements were collected.They were randomly divided into control group and experimental group.In both groups,the same group of doctors were selected to perform PVP surgery,which was performed by injecting bone cement into the unilateral pedicle approach.(1)the control group: on the third day postoperatively in patients with routine osteoporosis with taking the drugs,calcium carbonate D3 tablets(trade name calcium,),(manufactured by suzhou wyeth,60 per bottle,each containing 60 international unit dimension D3,750 mg of calcium carbonate,approved by: H10950030),1 slice/second,the bid of oral,a month for a period of treatment,taking four consecutive period of treatment.Forty-five cases were treated,4 cases fell off,and 41 cases were actually completed.(2)The experimental group: on the basis of the control group,the traditional Chinese medicine prescription of Bushen Zhuanggu Decoction was added and subtracted,one month was a course of treatment,and the Chinese medicine was taken continuously for four months,a total of four courses of treatment.45 cases were treated,5 cases fell off,and 40 cases were actually completed.A total of 81 cases were studied in the two groups.VAS scores and Oswestry Disability Index(ODI)scores were observed and recorded in detail before,3 days,2 months and 4 months after surgery.Bone mineral density(BMD)measurements were recorded before,2 months and 4 months after surgery,and vertebral height loss before and 4 months after surgery.Data on non-traumatic vertebral refractures were recorded during the first year after surgery.Finally,SPSS26.0 was used for statistical analysis of all data.Results:1.the two groups were analyzed and summarized by the basic data of gender,age,body mass index BMI,specific vertebral injury site,initial bone density before operation,preoperative VAS score,0di index evaluation,and vertebral compression severity.There was no statistical significance(P > 0.05),and the study was feasible.2.Oswestry Disability Index and VAS score;There was no significant difference in VAS score and ODI score between the TCM group and the reference group 3 days after surgery(P > 0.05).Two and four months than operation,the VAS scores and ODI scores in the Bushen Zhuanggu Decoction group and the reference object were compared,and the otherness was statistically significant(P< 0.05).The results showed that the postoperative VAS score and ODI score of both the traditional Bushen Zhuanggu Decoction group and the reference object were so much the better than those of the preoperative object,and with the extension of cure time,the effect of the experimental group was significantly better than that of the reference group.The results tested that the perfectment of pain motor and pain dysfunction in the experimental group were better than that in the control group.And with the advance of treatment time,the treatment effect is more and more obvious.3.Bone mineral density: the bone mineral density levels of the two groups were improved compared with those before treatment.There was no significant difference in bone mineral density measurement between the two groups 2 months after operation(P > 0.05),and there was significant difference in bone mineral density measurement between the two groups 4months after operation(P < 0.05).It means that the effect of the experimental group on improving the level of bone mineral density is higher than that of the reference group.4.the height of adjacent vertebral body;the height of adjacent vertebral body was statistically analyzed before and after operation in two and four months after operation.Independent sample t test was used to test the change of vertebral height before and after operation(P >0.05).The change of four months after operation was statistically significant(P < 0.05).5.the recurrence rate of fracture and the incidence of adjacent fracture recurrence after disposion.From the treatment to the follow-up period of 12 months after disposion,9 cases of vertebral body fracture happened in 41 cases in the reference team,and the recurrence rate was21.95%,of which 8 cases were adjacent vertebral fractures,with a incidence rate of 19.51%.In the experimental group,40 cases were found,only 1 cases had vertebral fractures again,and the incidence rate of adjacent vertebral fractures was 2.5%.The follow-up data of the number of vertebral refractures and adjacent vertebral fractures between the two groups were statistically significant(P < 0.05).It shows that Bushen Zhuanggu decoction can effectively reduce the incidence of vertebral refracture after PVP.Conclusion: Bushen Zhuanggu decoction can effectively relieve the residual spinal pain after PVP,improve the quality of life of patients,reduce the loss about vertebral height,and importantly reduce the danger of adjacent vertebral secondary fracture.It have a more-reaching effect on the clinical prevention of adjacent secondary fracture of the vertebral,which is worthy of further promotion. |
