Objective:To explore the changes of Th17,Treg and other indicators in peripheral blood of newly diagnosed AITD patients, and analyze their differences in HT and GD patients,so as to explore the role of Th17/Treg balance in the occurrence and development of AITD.Methods:Pubmed,Embase,CNKI,Wanfang Database and VIP Database were searched by computer, and the search time was from January 10 th,2021.English search terms are: "Autoimmune thyroid disease or thyroiditis,lymphocytic","Th17 cell or t helper 17 cell or type 17 helper t cells","T Lymphocytes,Regulatory or Cell,Treg or Th3 Cells or Suppressor T Cells,Naturally Occurring” 、 FOXP3.Chinese search formula:" Autoimmune thyroid disease or AITDS ",TH17,Treg,FOXP3 ".The relevant Meta were extracted and meta-analyzed by Rev Man 5.3 software. Results:A total of 28 case-control studies were included.Meta-analysis showed that Th17 levels in HT patients and GD patients were higher than those in healthy controls(P<0.00001,P < 0.0001).Treg(P=0.0005)(P=0.0007)and FOXP3 m RNA levels in HT patients and GD patients were generally lower than those in healthy controls(P<0.00001)and(p < 0.00001).Comparing the levels of Th17 in peripheral blood between HT patients and GD patients,it was found that the levels of Th17 in HT patients were higher than those in GD patients(P<0.00001).The frequencies of foxp3rs37615489 "cc" and "CT" genotypes in patients with HT are changed, which may be related to disease susceptibility. Conclusion:In newly diagnosed patients with autoimmune thyroid diseases, the increase of Th17 level and the decrease of Treg and FOXP3 m RNA levels lead to the imbalance of Th17/Treg level, which may be involved in the occurrence and development of the disease. The Th17 levels in peripheral blood of HT patients were higher than those of GD patients, which indicated that the two diseases had different immune reaction degrees, and their pathogenesis and immune system disorders were also different, including the difference of driving inducing factors and executive cytokines, which led to different outcomes of the two diseases.Detection of Th17/Treg level may indicate the immune steady state of the body.Th17/Treg may regulate the levels of inflammatory cytokines in AITD patients, and play an important role in its pathogenesis,so as to provide a new direction for clinical diagnosis and treatment.
