| Objective: Based on the medical big data platform of the first hospital of Shanxi Medical University,through the statistical analysis of the patient’s case data,this paper discusses cystatin C(Cystatin C,Cys C),α1-microglobulin(α1-Microglobulin,α1-MG)、β2-microglobulin(β2-microglobulin,β2-mg),transferrin(transferrin,TRF),neutrophils / lymphocytes(neutrophils / lymphocytes,NLR),these five indicators for the early diagnosis of diabetic nephropathy value.Methods: This study is a cross-sectional study.Using the scientific research big data platform of the first hospital of Shanxi Medical University,we searched 2973 patients with type 2 diabetes diagnosed in the Department of endocrinology of the first hospital of Shanxi Medical University from July 31,2007 to July 31,2017,and extracted the clinical data of the patients,Then,according to the urinary albumin to creatinine ratio(UACR),the patients were divided into three groups: simple diabetes group(group A),microalbuminuria group(group B)and massive proteinuria group(Group C).The age,gender,BMI,systolic blood pressure,diastolic blood pressure,TRF,Cys-C,α1-MG、β2-mg,NLR,to evaluate whether there are differences between groups,and then evaluate the correlation between the indicators with differences between groups and UACR,and finally analyze the value of these indicators in the early diagnosis of diabetic nephropathy(diabetic nephropathy,DN)on the basis of the above.Results:1.Chi square test showed that there was no significant difference in gender among simple diabetes group,microalbuminuria group and massive albuminuria group.Univariate analysis of variance showed that there was no significant difference in age,BMI,systolic blood pressure and diastolic blood pressure among simple diabetes group,microalbuminuria group and quantitative albuminuria group α 1-MG、β2-mg and NLR levels(F = 13901027611684752676183063,p < 0.05).2.The serum TRF,Cys-C,α1-MG、β2-mg,NLR and UACR showed that UACR was associated with serum TRF(r = 0.57,p < 0.01),Cys-C(r = 0.88,p < 0.01)and UACR βThe level of 2-mg(r = 0.85,p < 0.01)was positively correlated with that of serum α1-mg(r =-0.85,p < 0.01)and NLR(r =-0.71,p < 0.01)were negatively correlated.3.Receiver operator characteristic curve(ROC),β 2-mg single diagnosis,TRF+ β 2-mg,TRF + Cys-C+ β 2-mg was the highest(AUC = 0.796,YI = 0.59),and the difference was statistically significant(p < 0.01).Conclusion: Considering the cost and convenience of clinical work,we can β 2-mg single test is the best for early diagnosis of diabetic nephropathy,β 2-mg single diagnosis,TRF+ β 2-mg,TRF + Cys-C+ β 2-mg had high diagnostic performance,but serum TRF、Cys-C、α1-MG、β2-mg and NLR,whether alone or combined,have certain auxiliary diagnostic value for early diagnosis of diabetic nephropathy. |
