Objective To analyze depression in middle-aged and elderly adults and its correlation with overweight/obesity.Methods This study included 10024 residents from the baseline data of“REACTION”study of Lanzhou area.According to the body mass index(BMI),the subjects were divided into underweight(BMI<18.5kg/m~2),normal weight(18.5kg/m~2≤BMI<24.0kg/m~2),overweight(24.0kg/m~2≤BMI<28.0kg/m~2)and obese(BMI≥28.0kg/m~2).According to the waist circumference(WC),they were divided into the non-abdominal obesity(WC<90cm in males,WC<85cm in females)and the abdominal obesity(WC≥90cm in males,WC≥85cm in females).The depression status of study population was evaluated by the 9-item Patient Health Questionnaire Depression Scale(PHQ-9).The subjects were divided into non-depressed(PHQ-9<5points),subthreshold depression(5≤PHQ-9<10points),and probable depression(PHQ-9≥10 points)according to the scale scores,analyzing and comparing the basic characteristics of different depression condition and the prevalence of depression in different BMI groups and WC groups.Spearman correlation analysis was used to investigate the correlation between depression status and clinical data.Binary Logistic regression analysis was used to investigate the risk factors of subthreshold depression and probable depression,respectively.Results(1)The prevalence of subthreshold depression of the study population was12.4%(8.6%of male and 13.9%of female),and probable depression was 2.7%(1.9%of male and 3.0%of female).The prevalence of overweight was 39.0%(44.0%of male and 37.0%of female),obesity was 11.3%(12.7%of male and 10.7%of female),and abdominal obesity 45.3%(50.7%of male,43.1%of female).(2)By comparing the characteristics with different depression condition,it was found that whether subthreshold depression or probable depression,the proportion was higher in women than in men;unmarried,divorced and widowed was higher than that of married persons;people living alone was higher than that of non-single people;people having poor sleep or taking sleeping pills was significantly higher than that of good sleep.As physical activity increased,the prevalence of subthreshold depression decreased.Low level of physical activity was associated with the highest prevalence of probable depression,followed by high level of physical activity,and the lowest with moderate level of physical activity.There were no significant differences in the prevalence of depression among different age stratification,different educational level,whether having job,diabetes or dyslipidemia.(3)In this population,the prevalence of subthreshold depression was 17.4%,13.3%,11.4%and 10.6%in underweight,normal weight,overweight and obese people,respectively.The prevalence of probable depression was 4.0%,3.0%,2.5%and 1.8%,respectively.The depression score increased with the increase of BMI(all P<0.05).The subthreshold prevalence of depression in abdominal obesity was 11.4%,which was lower than that in non-abdominal obesity(P<0.05),and the prevalence of probable depression was 2.5%,which was not significantly different from that in non-abdominal obesity(P>0.05).(4)Depression status were negatively correlated with BMI and WC,and the correlation coefficients were-0.054 and-0.053,respectively(all P<0.05).(5)Logistic regression analysis showed that compared with the normal weight group,the risk of subthreshold depression in the obese group was decreased(OR=0.791,95%CI:0.639~0.980),and the risk of subthreshold depression in the underweight group was increased(OR=1.422,95%CI:1.007~2.007)after adjusting for various confounding factors.Compared with the non-abdominal obesity group,the risk of subthreshold depression was lower in the abdominal obesity group(OR=0.875,95%CI:0.770~0.993).Overweight,obesity,and abdominal obesity were not associated with the risk of probable depression(all P<0.05).Conclusion1.The prevalence rate of subthreshold depression and probable depression among middle-aged and elderly people in Lanzhou was relatively high,among which women,unmarried,divorced,widowed,people living alone,having poor sleep and people with low physical activity had higher tendency of depression.2.Overweight was not associated with the risk of subthreshold depression,Obesity and abdominal obesity were negatively correlated with the risk of subthreshold depression,while underweight was positively correlated with the risk of subthreshold depression,which was a risk factor for the occurrence of subthreshold depression.Overweight,obesity,and abdominal obesity were not associated with the risk of probable depression.
