Objective:To explore the effects of budesonide combined with low-dose dexamethasone on blood gas analysis,inflammation indicators,and hospital stay in patients with pulmonary contusion,and to evaluate the clinical value of the combined application of the two drugs in patients with pulmonary contusion.Methods:The 103 patients with lung contusion admitted to the Department of Thoracic Surgery of Qinghai Provincial People’s Hospital from November 2019 to November2020 were selected as the research objects.Upon admission,the patients were included in different treatment groups according to the principle of randomization,with 55 cases in the observation group and 48 cases in the control group.Control group:add a small dose of hormones to the basic treatment,observation group:add budesonide atomized inhalation to the control group.Compare the difference in treatment effect between the two groups of patients.(Including Pa O2,Sa O2,Pa CO2,PCT,IL-6,hs-CRP and hospitalization days in the two groups of patients at different treatment time points).Results:1.Comparison of basic data(gender;age;type of injury;ISS score;Sa O2,Pa O2,Pa CO2,PCT,IL-6,hs-CRP at admission,whether it is combined with rib fractures,whether or not to perform thoracic drainage)No statistical significance(P>0.05).2.The comparison of Sa O2 and Pa O2 between the two groups of patients at different treatment time points was different(P<0.05),and Sa O2 and Pa O2 in each group showed an upward trend with the increase of treatment time.There was a difference in Pa CO2 comparison between the two groups on the 3rd,5th,and 7th day after treatment(P<0.05),and there was no difference in Pa CO2 comparison between the two groups on the first day after treatment(P>0.05).The treatment within each group followed Pa CO2 showed a downward trend with the increase of time(P<0.05).After 7 days of treatment,the patient’s blood gas indicators were significantly better than those at admission,and the observation group was significantly better than the control group.3.There are differences in IL-6 comparison between the two groups of patients at different treatment time points(P<0.05),and there are differences in the comparison of PCT and hs-CRP between the two groups on the 3rd,5th,and 7th days after treatment(P<0.05).On the first day after treatment,there was no difference in PCT and hs-CRP between the two groups(P>0.05).With the increase of treatment time,PCT,hs-CRP and IL-6 in each group increased first and then decreased(P<0.05),the peak value is reached on the 3rd day after treatment.After 7 days of treatment,the inflammatory index of the patient was significantly lower than that at admission,and the observation group was significantly better than the control group.The difference in hospital stay between the two groups was statistically significant(P<0.05).The average stay in the observation group was less than that in the control group.4.The difference in hospital stay between the two groups was statistically significant(P<0.05),and the average stay in the observation group was less than that of the control group.Conclusion:1.The inflammatory indexes of patients in both groups showed a trend of first increasing and then decreasing,reaching the peak on the third day after treatment and then beginning to decrease.2.Through clinical treatment,the blood gas indexes of the two groups were significantly improved,and the indexes of the observation group were significantly better than the control group on the 7th day after treatment.3.Budesonide combined with dexamethasone has a good effect on the control of inflammatory indexes and the improvement of blood gas indexes in patients with pulmonary contusion,and the combined application of the two drugs can reduce the average hospital stay of patients.
