Objective:To explore the effect of restricted resuscitation with sodium bicarbonate Ringer’s solution on the internal environment of patients with traumatic hemorrhagic shock to evaluate the resuscitation effect of sodium bicarbonate Ringer’s solution.Methods: This experiment uses a prospective controlled study method.Enrolled 48 patients with traumatic hemorrhagic shock in the emergency department of the First Affiliated Hospital of Bengbu Medical College from March 2019 to June 2020 as the research objects.The patients were randomly divided into two groups(observation group and control group),each group was 24 patients.Among them,the observation group was resuscitated with bicarbonate Ringer’s solution(bicarbonate Ringer’s solution),and the control group was resuscitated with lactate Ringer’s solution.Both groups of patients were resuscitated with restrictive fluids.The peripheral blood and arterial blood of the two groups of patients were drawn before resuscitation and 1hour after resuscitation,and the levels of IL-4,IL-6,IL-10,TNF-α in peripheral serum,peripheral platelet count,and plasma thrombin were measured Original time,fibrinogen level and arterial blood lactate value.By comparing the changes of the above indicators between the two groups of patients,the effect of restricted resuscitation with sodium bicarbonate Ringer’s solution on the internal environment of traumatic hemorrhagic shock patients was observed.Results:Before resuscitation,there was no significant difference in the above indicators between the two groups of patients(P>0.5).After 1 hour of resuscitation,the levels of IL-4,IL-6,IL-10,and TNF-α in peripheral blood of the two groups increased.The difference in the range of increase was performed by paired-sample t-test,and the results showed that the difference in the range of increase in the levels of peripheral blood inflammatory factors between the two groups was statistically significant(P<0.05).It shows that Sodium Bicarbonate Ringer’s Solution can inhibit inflammation.Before resuscitation,the arterial blood lactic acid value of the two groups increased.After 1 hour of resuscitation,the lactic acid value of the two groups were lower than before resuscitation,but still higher than the normal value.There was a statistically significant difference in the magnitude of their decrease(P<0.05).Shows that sodium bicarbonate Ringer’s solution can improve acidosis.One hour after resuscitation,the platelet count,plasma prothrombin time,and fibrinogen levels of the two groups were improved,but there was no significant difference between the groups(P>0.5).Conclusion:Compared with sodium lactate Ringer’s solution,the application of sodium bicarbonate Ringer’s solution in the early stage of traumatic hemorrhagic shock can better inhibit the systemic inflammatory response,reduce the damage of excessive inflammation to the body,and at the same time can better inhibit acidosis.It shows that sodium bicarbonate Ringer’s solution is more suitable as a resuscitation fluid in the early stage of traumatic hemorrhagic shock.
