Study On The Correlation Between Cerebral Oxygen Saturation And PEEP Level Of Thoracoscopic Elderly Patients With Lung Cancer

Objective:To investigate the correlation between end-expiratory pressure level and cerebral oxygen saturation in thoracoscopic elderly patients with lung cancer.Methods:According to inclusion and exclusion criteria,sixty elderly patients undergoing thoracoscopic lobectomy in the the first affiliated Hospital of Bengbu Medical college from October 2019 to February 2020 were selected,They were divided into three groups according to the order of admission and 20 cases of each groups.The tidal volume（V_T）in group A,group B and group C was set as 5ml/kg of the basic body weight,PEEP was set as 0,5,10cmH₂O during one-lung ventilation respectively.Pulse oxygen saturation（SpO₂）,peak airway pressure（Ppeak）,pulmonary dynamic compliance（Cydn）,arterial oxygen partial pressure（PaO₂）,cerebral oxygen sturation（rSO₂）were recorded at15min after anesthesia induction（T₀）,10 min（T₁）,30min（T₂）,60min（T₃）after one-lung ventilation and 20min after recovering two-lung ventilation（T₄）.Results:Compared with group A（66.65±2.72,69.15±4.27）and group C（71.70±6.04,73.75±2.15）,rSO₂min and rSO₂avgingroup B（71.70±4.50,76.40±2.66）was increased significantly（P<0.05）,compared with group A and group C（13.95±4.16,11.45±2.72）,rSO₂max%in group B（8.90±1.77）was decreased significantly（P<0.05）.At T₁-T₄time points,rSO₂of group B was higher than of group A and group C,in which group C was higher than group A（P<0.05）.Conclusion:The changes of cerebral oxygen saturation in elderly patients undergoing lung resection for lung cancer are corrlated with PEEP levels,5cmH₂0PEEP has better rSO₂level.
