Research On The Performance Of Jiangxi Province’s Medical And Health Fiscal Expenditure From The Perspective Of Efficiency And Equity

As a basic project to solve the problem of "national livelihood",medical and health services play an important role in improving our country’s social security system.Since the introduction of the medical and health reform policy in 2009,Jiangxi Province has continuously increased the scale of medical and health fiscal expenditures and is committed to promoting the fairness of medical and health services.However,the efficiency and equity of medical and health fiscal expenditures need to be further evaluated.The outbreak of the new crown epidemic not only endangers the lives and health of our people,but also has a huge impact on our economy.At the same time,as a major public health event,it also tests the emergency response capabilities of our public health system.Based on the new crown pneumonia epidemic,a public health event,this article studies the performance of Jiangxi Province’s medical and health fiscal expenditures from a new perspective,and explores the problems and solutions in Jiangxi Province’s medical and health fiscal expenditures.This paper firstly analyzes the concepts and theories related to medical and health fiscal expenditure,and analyzes the current situation of medical and health fiscal expenditure and resource allocation and use in Jiangxi Province from the aspects of expenditure scale and structure,health resource allocation and utilization,and draws the conclusion that Jiangxi Province’s medical and health expenditure investment is insufficient and the structure is unreasonable.Then it analyzes the performance level of Jiangxi’s medical and health fiscal expenditure from the perspectives of efficiency and equity,and uses the DEA-BBC model and the DEA-Malmquist productivity index to measure the static and dynamic efficiency of Jiangxi Province’s medical and health fiscal expenditure from 2009 to 2018.Based on a comprehensive analysis of its efficiency,the results show that the overall efficiency of Jiangxi Province’s medical and health fiscal expenditures from 2009 to 2018 is relatively high,but there are differences among cities at various levels.The lack of technological progress and innovation has led to a decline in total factor productivity;using the Tobit model Analyzed the external factors affecting the efficiency of Jiangxi’s medical and health fiscal expenditures,and found that population density,urbanization rate,and fiscal decentralization have a positive impact,and the proportion of medical and health fiscal expenditures has a negative impact,and the impact of per capita GDP is not significant;use Tyre index method measures the fairness of medical and health fiscal expenditures and the allocation of the three major health resources in Jiangxi Province from 2009 to 2018.The results show that in addition to the deterioration of the fairness of health institutions,the fairness of the number of health technicians,the number of beds in health institutions,and the medical and health fiscal expenditure has been improved,and interarea differences are the main reasons that affect the equity of medical and health fiscal expenditures,the number of health technicians,and the number of beds in health institutions in Jiangxi Province.The differences within the area are the main reasons that affect the equity of the allocation of health institutions in Jiangxi Province.Finally,a series of policy recommendations to improve the performance of medical and health fiscal expenditures in Jiangxi Province were proposed,including increasing medical and health fiscal expenditures,adjusting the structure of medical and health fiscal expenditures,advancing the process of urbanization,overall planning of the province’s medical and health resources,strengthening the construction of medical and health personnel,and exerting the audit and supervision and management functions.
