| Objective : To explore the correlation between the baseline serum HER2(serum human epidermal growth factor receptor-2,s HER2)expression level,the trend of s HER2 expression level after treatment and the curative effect and prognosis in patients with advanced breast cancer.Methods:Dynamic detection and grouping of baseline s HER2 expression levels and changes in s HER2 after treatment in 200 patients with advanced breast cancer admitted to the Tianjin Medical University Cancer Institute & Hospital from July2016 to June 2016.And their clinical data were collected and retrospectively analyzed.They were grouped according to the baseline s HER2 expression level and changes in s HER2 after treatment.The data of clinical pathological characteristics,treatment efficacy and the prognosis were statistical analysed,so as toobtain the correlation between the baseline s HER2 expression level,the trend of s HER2 expression level after treatment and the curative effect and prognosis in patients with advanced breast cancer.Applicate SPSS 25.0 software to analyse.Chi-square test was used for comparison of count data,and Kaplan-Meier method was used for survival analysis.P value <0.05 indicates that the difference is statistically significant.Results:1.Use 15 ng / ml as the cut-off value to define high or low level of s HER2 expression.The high expression of s HER2 is significantly related to molecular classification(p=0.016),visceral metastasis(p<0.001)and liver metastasis(p<0.001).2.When breast cancer patients relapse / metastasis,the objective response rate(ORR)of patients with high expression of s HER2 at baseline is higher than that of patients with low expression of s HER2(p=0.026);the time to progression of patients with low expression of s HER2 at baseline is longer than the TTP of patients with high expression of s HER2 at baseline(p=0.024).3.A decrease of 20% or more from baseline is defined as a significant decrease.A significant decrease in s HER2 after two cycles of treatment indicates a significant curative effect.For patients with high expression of baseline s HER2,a significant decrease in s HER2 after two cycles of treatment indicates an extension of TTP.4.In patients with high expression of s HER2 at baseline,s HER2 drops to negative after treatment suggesting higher ORR and prolonged TTP.5.Regardless of immunohistochemical human epidermalgrowth factor receptor-2(i HER2)status is positive or negative,high s HER2 expression can occur during relapse / metastasis.6.For patients with low expression of baseline s HER2 and negative i HER2,s HER2 remains negative during treatment indicates a significant curative effect,but no extension of TTP has shown.7.s HER2 expression decreased by 50% or more from the previous cycle indicatites that the tumor remission rate was higher,but TTP was not prolonged.s HER2 expression level increased steadily indicates a poor curative effect,and s HER2 expression level increased rapidly indicates that TTP shortened.Conclusion:For patients with advanced breast cancer,whether it is i HER2 positive or i HER2 negative,dynamic detection of s HER2 is conducive to the prediction of curative efficacy and play certain auxiliary role in prognosis. |
