| Objective: The aim of the study was to explore the long-term quality of life(Qo L)and the incidence of metabolic syndrome(MS)in patients with polycystic ovary syndrome(PCOS),and analyze the related factors of them to guide clinical treatment.Methods: Collected and recorded the clinical data of patients who was diagnosed as PCOS and established files in the Department of Obstetrics and Gynecology,Tianjin Medical University General Hospital from January 2000 to January 2015.The patients were followed up by telephone or on-site follow-up,the recent general condition and the incidence of MS were recorded.The quality of life of the patients in the past two weeks was evaluated by Modified Polycystic Ovary Syndrome Health Related Quality of Life Questionnaire(MPCOSQ).Calculated and compared the average scores of all dimensions of patients with different course and age,analyze the relationship between current indicators and Qo L.The patients were divided into groups according to whether the patients had abnormal metabolism at diagnosis.Results: 1.A total of 260 patients were included and 221 patients were followed up successfully.The average age of the patients was 23.13±5.43 years old at diagnosis,and was 32.82 ± 6.87 years old currently.There was no significant difference in body weight,BMI and abdominal circumference.2.The total score of MPCOSQ was 133.83±24.33,and the dimensional average score was 4.46±0.81.The dimensional average score from low to high were weight,emotions,menstrual predictability,infertility,body hair,menstrual symptoms,acne.The score of menstrual predictability was the lowest among the patients aged 18-25,and the score of hairiness was also lower.Patients aged over 25 had the lowest score of weight,followed by infertility in people aged 25-35,and menstrual predictability over 35 years old.Patients with longer duration of disease had lower score.3.Single factor analysis showed that abdominal circumference,BMI,hirsutism,acanthosis nigricans,menstruation and fertility were the influencing factors of Qo L in PCOS patients.Multiple linear regression analysis showed that BMI,hirsutism,acanthosis nigricans,menstruation and fertility condition were the main factors of Qo L in PCOS patients(P<0.05).4.The 146 patiens without metabolic abnormality at diagnosis were divided into groups by the incidence of MS at the time of follow-up.Single factor analysis and logistic regression analysis showed that the abdominal circumference、BMI,serum total testosterone,HOMA-IRand the duration of the disease were the independent risk factors for the incidence of MS(P<0.05).5.The analysis of the 75 patients with metabolic abnormality at diagnosis found that the patients with long-term metabolic reversed to normal were younger in onset age and shorter in duration.And logistic regression analysis showed the onset age was the independent risk factor for the reversion of MS(P<0.05).6.Compared with PCOS patients without MS,patients with metabolic abnormality but not enough to diagnose MS and patients with MS,the MPCOSQ of the three groups decreased significantly(F = 4.722,P < 0.010).Conclusion: 1.The four main dimensions affecting the Qo L of PCOS patients are weight,emotionas,menstrual predictability and infertility.The influence degree of each dimension on the Qo L varies with the length and age of the patients.The score of MPCOSQ in patients with long duration of disease is lower.The adolescent patients pay more attention to menstruation and body hair.Women at childbearing age show solicitude for weight and infertility.2.BMI,body hair,acanthosis nigricans,irregular menstruation and infertility mainly affect the Qo L of PCOS patients.Metabolic syndrome is also an important factor affecting the quality of life.3.Abdominal circumference,BMI,serum total testosterone,HOMA-IR at diagnosis and the duration of the disease are the independent risk factors for the incidence of MS in PCOS patients.4.For PCOS patients with metabolic abnormalities at the early stage of the disease,the younger the age of onset and the shorter the duration of disease,the more likely the long-term MS will be reversed. |
