| Objective:To explore the clinical effect of laparoscopic partial splenectomy(LPS)in patients with traumatic splenic rupture(TSR).Methods:Through retrospective analysis of the relevant clinical data of 25 patients with traumatic splenic rupture who underwent laparoscopic surgery at the Department of Hepatobiliary and Pancreatic Surgery of the First People’s Hospital of Zigong City from February 2017 to July 2020.According to the different surgical methods,patients were divided into laparoscopic partial splenectomy group(12 cases)and laparoscopic splenectomy group(13 cases).The operations of the two groups of patients were performed jointly by the attending physician or above with a senior professional title,and the camera systems of the two groups of operations were the German STORZ IMAGE 1 S high-definition camera system.Finally;compare the general conditions of the two groups of patients in the perioperative stage and related indicators such as postoperative complications,and analyze the results.Results:Both groups of patients successfully completed the operation according to the selected surgical method,and there was no conversion to open surgery,intraoperative change of surgical method,occurrence of postoperative serious complications and death cases.The gender,age,average duration of injury,preoperative hemoglobin,and the severity of the spleen injury were roughly the same in the two groups of surgical patients,and the difference was not objectively statistically significant(P>0.05).The operation time of the patients in the LPS group and the LS group were 178.75±48.15 minutes and 178.46±41.05 minutes,respectively.The intraoperative blood loss of the two groups were 234.17±125.51ml and 230.77±161.42ml,respectively.By comparison,there was no statistically significant difference in the operation duration and intraoperative blood loss between the two groups of patients(P>0.05).However,the first postoperative anal exhaust time in the LPS group was 1.69±0.48(d),hospitalization time was 8.50±1.45(d),and the total cost of 30541.17±3741.15(yuan)was lower than that of the LS group,and the difference was statistically significant(P<0.05).Conclusion:Compared with laparoscopic splenectomy,laparoscopic partial splenectomy has less damage,lower incidence of postoperative complications,and faster recovery for the treatment of grade Ⅰ-Ⅲ splenic injury and hemodynamically stable traumatic splenic rupture.Several major advantages such as retaining spleen function are worthy of further research,promotion and application in surgical clinics. |
