Objective:To explore the artificial creation of wounds to simulate surgical incisions,to study the effect of simple intermittent suture combined with multi-point intradermal microinjection of polymer cross-linked hyaluronic acid on early scar formation of incisions,and to provide theoretical basis and operating methods for clinical scar improvement.Methods:In the experiment,New Zealand rabbits with no statistical difference in size,weight and growth status were used as animal models.The experiment was divided into two groups.The skins were prepared and disinfected at the same position on the back of the rabbits,and linear wounds were made at the scheduled incisions and deep into the skin.Immediately inject 1ml of polymer cross-linked hyaluronic acid intracutaneously,and then suture the skin intermittently.Compared with another group of symmetrical injection of 1ml of saline and then routinely interrupted suture to compare the scar formation after healing.The specific experimental methods are as follows:1.Take 36 8-week-old female New Zealand rabbits,and randomly select the left side of the back of the rabbit as the research object.The experimental group is marked as group A,and the control group is marked as group B,with 18 animals in each group;2.Sterilize skin preparation,design a 10 CM linear incision on the left side of the rabbit’s back.3.After the free operation area was completely hemostasis,the experimental group injected 1ml of polymer cross-linked hyaluronic acid(Haiwei)into the back skin of rabbits until small skin hills appeared,and each small skin hill was injected about0.05 ml,each skin The mound spacing is 2cm.Repeat simple interrupted suture;4.The control group was injected with 1ml of saline,and the other operations were the same as the experimental group;5.Disinfect both sides at the same time after the operation,and bandage the wound.Routine dressing of the wound was changed 3 and 7 days after surgery to prevent infection.6.The samples were taken 7 days,14 days,and 28 days after injury.The sampling site is a 1.5cm square skin tissue centered on the incision,and the tissue level is from the epidermis to the deep layer of the superficial fascia on the back of the rabbit.Through the observation of HE staining under the microscope of pathological section and the detection of α-SMA,masson staining and the content of hydroxyproline,the wound healing and scar hyperplasia were evaluated.7.Finally,use SPSS23 statistical software to process the data.Result:1.There is no obvious difference between the two in the wound healing process after7 days from the naked eye.After 14 days and 28 days,especially after 28 days,it is obvious that the wound healing(area value)of the experimental group is compared with the scar tissue area of the control group(Value)is smaller,the scar texture is more delicate,but there is no obvious difference in the color of the scar between the two,all are white linear scar tissue;2.Observed under a 20X10 magnification microscope: 14 days and 28 days HE staining can see that the number of macrophages in the experimental group is less than the control group,the number of capillaries is relatively small,and the arrangement of fibroblasts is relatively regular;3.Observing masson staining under the microscope at 20x10 times: The ratio of the collagen surface density under the light microscope at 7 days,14 days and 28 days in the control group is: X1w=18.55±3.26%,X2w=23.97±4.57%,X4w=43.44±7.28%,the collagen surface density ratio of the experimental group at 7 days,14 days and 28 days under light microscope was X1w=12.87±1.02%,X2w=16.81±1.35%,X4w=30.71±2.98%.The t test compared the relative content of collagen between the two groups at 7,14 and 28 days,P<0.05,the difference was statistically significant;4.Observe the α-SMA staining under the microscope at 20x10 times: the average optical density of α-smooth muscle actin under the light microscope at 7 days,14 days and 28 days in the control group is X1w=0.0238±0.0 023 OD,X2w=0.0367±0.0035 OD,X4w=0.0447±0.0041 OD,the average optical density of α-smooth muscle actin under the light microscope in the experimental group at 7 days,14 days and 28 days was X1 w = 0.0151±0.008 OD,X2w=0.0304±0.0017 OD,X4w=0.0371 ±0.0023 OD.The t test compared the average optical density of α smooth muscle actin at 7,14 and 28 days,P<0.05,the difference was statistically significant;5.The content of hydroxyproline in the control group(X1w=2.806±0.232 OD,X2w=3.965±0.319 OD,X4w=4.506±0.387 OD)was greater than the polymer hyaluronic acid group at 1w,2w and 4w(X1w= 2.245±0.158 OD,X2w=2.698±0.264 OD,X4w=3.105±0.373 OD),the t test compared the hydroxyproline content of each group,the difference was statistically significant P<0.05.In conclusion:The application of high molecular crosslinked hyaluronic acid intragermal microinjection combined with intermittent suture can inhibit the rapid proliferation of early inflammatory cells in the back of New Zealand female rabbits,reduce the amount of scar formation in early wound healing,and thus improve the formation of early scar.
