Study On Depression Status And Correlation In Patients With Ankylosing Spondylitis

Objective:Ankylosing spondylitis(Ankylosing Spondylitis,AS)is a kind of chronic progressive inflammatory spinal arthritis.Compared with the general population,AS patients have an increased risk of depression,affecting their prognosis,which has attracted more and more attention.In this study,a cross-sectional survey was conducted to evaluate the status of depression in patients with AS,and to explore the relationships between depression and pain,fatigue,sleep quality,quality of life,as well as Bath Ankylosing Spondylitis Disease Activity Index(BASDAI),Bath Ankylosing Spondylitis Functional Index(BASFI)and Bath Ankylosing Spondylitis Metrology Index(BASMI)in patients with AS,in order to find out the important influencing factors of depression in patients with AS.Methods:50 outpatients and inpatients with AS in Department of Rheumatology and Immunology,China-Japan Union Hospital of Jilin University from November 2020 to February 2021 were investigated by questionnaire,including general demographic characteristics,Patient Health Questionnaire-9(PHQ-9),pain Visual analogue scale(pain VAS),Multidimensional Fatigue Inventory(MFI-20),Pittsburgh Sleep Quality Index(PSQI),health survey summary form(SF-36),BASDAI,BASFI,BASMI.The relationship between depression and pain,fatigue,sleep quality,quality of life,and BASDAI,BASFI,BASMI was analyzed by Spearman correlation analysis and linear regression analysis.Results:(1)The study found that the prevalence rate of depression in AS patients was66%,and the prevalence rate of moderate to severe depression was 24%,that is,24%of AS patients reached clinical depression.(2)Sex,age,BMI,occupation,marital status,education level,course of disease,smoking history and genetic history were not significantly correlated with depression in AS patients.(3)Night pain,general back pain,peripheral joint pain,fatigue,sleep quality,quality of life,BASDAI and BASFI were significantly correlated with depression in patients with AS.Fatigue has the strongest correlation with depression in patients with AS,while there is no significant correlation between BASMI and depression in patients with AS.(4)BASDAI,mental health level and fatigue were important influencing factors of depression in patients with AS,and BASDAI was a strong predictor of depression in patients with AS.Conclusion:AS patients are prone to mental health problems,general demographic factors and other background factors may have little impact on the occurrence of depression in AS patients.Pain,fatigue,sleep quality,quality of life and other disease-related factors are closely related to the occurrence of depression in AS patients.Clinicians should pay attention to the mental health problems of AS patients with higher BASDAI scores.
