Application Of The Technology Of Virtual 3D-CTBA Based On Computer On Precise Anatomical Segmentectomy

Objective: This study assesses the clinical value of three-dimensional CT bronchography angiography(3D-CTBA)combined with segmentectomy in the treatment of ground-glass nodule.Material and methods: In this study,a retrospective study was used to collect data of 65 patients who underwent the video-assisted thoracic lung segmentectomy in our hospital from July 2015 to October 2020.According to whether they voluntarily performed 3D-CTBA,3D-CTBA was performed before operations in 30patients[a observation group,including 13 males and 17 females with a median age of 51(44.75～60)] and not performed in other 35 patients [a control group,including 16 males and 19 females with a median age of 53(38～65)].Compared the differences of data between the two groups in the perioperative period.Results: The operations went successful for all the patients,and there were nocases of perioperative death,and all were discharged from the recovery.The basic data of the two groups were not statistically significant after comparison(P>0.05).There were significantdifferencesin operative time[130(95～141.25)min vs 140(120～160)min,P=0.017],blood loss [105(95～133)ml vs 195(100～257)ml,P=0.001],postoperative hospital stay [4(3～5)d vs 5(4～6)d,P=0.003],postoperative extubation time [3(2～4)d vs 4(3～5)d,P=0.001],postoperative total drainage[656.00(485.25～890.50)ml vs 862.00(596.00～1035.00)ml,P=0.044]between the two groups.Conclusion: Preoperative three-dimensional computed tomography bronchial angiography can be used to locate the lesion accurately.It is very useful to design a reasonable surgical plan,and reduce the operation time and blood loss by it.It has a certain effect on the postoperative recovery of the patient.The clinical application of 3DCTBA combined with precise lung segmentectomy is safe and effective.
