Objective:To compare the complication rate, satisfaction and quality of life of postoperative radiotherapy with different reconstruction methods for breast cancer, and to find the best combination of reconstruction and radiotherapy. Methods:The clinical data of 105 patients with breast cancer undergoing breast reconstruction and radiotherapy treated at the Tumor Hospital affiliated to the third Clinical Medical College of Xinjiang Medical University from 2014 to 2019 were enrolled. They were divided into group A(Radiotherapy after autologous reconstruction,54 patients)and group B(Radiotherapy after implantation reconstruction,51 patients)according to the reconstruction types.Group B was further divided into group B1(Radiotherapy after one-step reconstruction,30 patients)and group B2(Radiotherapy between two-step reconstruction,21 patients)according to the timing of reconstruction. The complication rate and BREAST-Q score of postoperative radiotherapy in group A and group B,group B1 and group B2 were compared, and the influencing factors of BREAST-Q score were analyzed by multiple linear regression model.Results:1)The incidence of minor complications in group A and group B were 24.1%and 45.1%,respectively,the incidence of long-term complications were 3.7%and 37.3%,and the incidence of total complications was 24.1%and 54.9%,respectively.The incidence of minor complications,long-term complications and total complications in group A were lower than those in group B,the difference was statistically significant(P=0.023;P<0.001;P=0.001).In group B,there was no significant difference in the incidence of various complications and total complications between group B1 and group B2(all P>0.05);2)The scores of the Psychosocial Well-Being of group A and group B are 71(15)and 66(22),respectively,and the scores of Satisfaction With Breasts are 53(8)and 53(8),respectively.The scores of Psychosocial Well-Being and Satisfaction With Breasts of group A was higher than that of group B,and the difference was statistically significant(P=0.027;P=0.032).In group B,there was no significant difference in the scores of the four BREAST-Q scales between group B1 and group B2(all P>0.05).3)The results of multiple linear regression showed that breast size(P<0.001)and complications(P<0.001)were the predictors of BREAST-Q score(P<0.001;R~2=0.516).Conclusion:Radiotherapy after autologous reconstruction of breast cancer has fewer complications than the implant reconstruction, and the BREAST-Q score is better. The complication rate and the BREAST-Q score of the radiotherapy after the one-step and two-step reconstruction of the implant reconstruction are equivalent; The BREAST-Q score of patients with big breasts or complications is poor.
