ObjectiveTo study the application of Buyang Huanwu Decoction to the treatment of "Qi deficiency and blood stasis" syndrome after hip fracture in elderly patients and to analyze the diagnosis and treatment rules.To observe the improvement of TCM clinical symptoms such as shortness of breath and fatigue,swelling of affected limbs and self-sweating before and after treatment,and to evaluate the coagulation function index and the occurrence of venous thrombosis in lower extremities.Scientific and objective evaluation of this method,in order to accept the lower extremity orthopedic surgery of the elderly patients,the use of Buyang Huanwu decoction treatment can obtain better clinical efficacy,And to supplement the third stage dialectical theory of TCM fracture in the environme nt of modern surgical technology,so as to further give full play to the special advantages of integrated Chinese and western medicine in the treatment of orthopedic related diseases.MethodsAll the clinical observation cases of Qi deficiency and blood stasis were taken from the elderly patients who were hospitalized in the lower extremity orthopedic department of Xiangyang traditional Chinese Medicine Hospital in July 2019~June 2020.And agreed to hip replacement or other hip fracture incision and internal fixation treatment.According to the inclusion and exclusion criteria,60 cases were selected and randomly divided into control group and treatment group.The patients in the control group were treated with routine symptomatic treatment,such as prevention of infection,anticoagulant,detumescence and pain relief,and the treatment group was on the basis of control group treatment,from the first day after the operation,treated with Buyang Huanwu decoction for 14 days.The blood coagulation function indexes,TCM symptom score,the effect of treatment and the difference of deep venous thrombosis were compared between the two groups before and after operation on the first day and 14 days after operation.Finally,the data of the research results are compared by statistical method.ResultsNo serious complications such as infection,shock and death occurred after operation.1.Comparison of coagulation function:There was no significant difference in the APTT、PT and FIB indexes of the two groups before operation,1 day after operation and 14 days after operation(P>0.05),and the D-dimer in the treatment group was significantly lower than that in the control group(P<0.05)14 days after operation.2.comparison of TCM symptom score: compared between the two groups,the symptoms of TCM after treatment were significantly reduced compared with those before treatment,and the scores were significantly reduced(P<0.05).Before and after treatment,there was no significant difference between the two groups(P>0.05).3.Comparison of clinical efficacy:The total effective rate of treatment group was 93.3%,which was better than 76.7% of the control group(P>0.05).4.The incidence DVT deep venous thrombosis was compared: no DVT was found in the two groups before operation,no DVT was found in the treatment group 1 day after operation and 14 days after operation,and 3 cases of intramuscular venous thrombosis occurred in the control group.There was no significant difference(P>0.05).ConclusionBuyang Huanwu decoction can effectively improve the symptoms of qi deficiency and blood stasis after hip fracture in elderly patients.The effect of this prescription can effectively prevent the formation of deep venous thrombosis of lower extremity,improve the therapeutic effect of inpatients and improve the safety of postoperative curative effect.This study shows that the combination of traditional Chinese and western medicine has special advantages in the treatment of fracture diseases.This study also contributes to the improvement of TCM fracture three-stage dialectical theory in modern surgical treatment environment.
