Objective: Proteinuria in diabetic nephropathy patients is an important factor affecting their prognosis and disease progress.The purpose of this study is to observe the improvement of proteinuria in diabetic nephropathy patients by ramipril and Keluoxin capaules,so as to explore a more effective treatment plan for diabetic nephropathy patients to control proteinuria.Methods: From October 2017 to October 2019,80 patients with diabetic nephropathy were selected for diagnosis and treatment in the First Affiliated Hospital of Hebei North University.they were divided into 40 patients with stage III early DN and 40 patients with stage IV clinical DN.80 patients were randomly divided into experimental group(ramipril combined with keluoxin capaules)and control group(ramipril alone).In the experimental group,there were 20 patients with early DN and 20 patients with clinical DN;in the control group,there were 20 patients with early DN and 20 patients with clinical DN.All patients were given routine treatment of diabetic nephropathy,including low salt and low fat diet,reasonable exercise,oral hypoglycemic drugs or subcutaneous injection of insulin to control blood sugar and ensure the intake of protein 0.6-0.8g /(kg?d),and stop anticoagulation,lipid regulation and ACEI or ARB drugs for at least 4 weeks.After blood pressure and blood glucose were controlled and stabilized for 2 weeks,clinical trials were carried out.The control group was given 5mg ramipril once a day,and the experimental group was given 2g keluoxin capsule three times a day on the basis of the control group.The FBG,BP,24-hour urinary protein,plasma albumin and serum creatinine were measured before and 8 weeks after the treatment.Results: 1.There was no significant difference in BP and FBG between the experimental group and the control group before and after treatment,and the differences were not statistically significant(P>0.05).2.For early DN,the urinary albumin excretion rate of the experimental group and the control group decreased compared with that before the treatment,and the difference was statistically significant(P <0.05),and the urinary albumin excretion rate of the experimental group decreased after the treatment compared with the control The group was more obvious,and the difference was statistically significant(P <0.05).3.For early DN,compared with before treatment,there was no significant difference in serum albumin and creatinine in the control group(P > 0.05);there was no significant difference in serum albumin and creatinine in the experimental group(P > 0.05).4.For clinical DN,the 24-hour urinary protein quantification in the experimental group and the control group decreased compared with before treatment,and the difference was statistically significant(P <0.05),and the 24-hour urinary protein quantification in the experimental group decreased after treatment compared with the control The group was more obvious,and the difference was statistically significant(P <0.05).5.For clinical DN,compared with before treatment,there was no significant difference between the control group and the experimental group(P > 0.05);there was no significant difference between the experimental group and the experimental group(P > 0.05).6.During the treatment,the patients all had serious adverse reactions.Conclusions: 1.For patients with early DN and clinical DN,the therapeutic effect of ramipril combined with keluoxin capsules on proteinuria is better than ramipril alone.2.No serious adverse reactions occurred during the treatment.
