Objective:In this study, the data of patients with enlarged cervicodorsal fat pads were reviewed comprehensively to summarize the clinical characteristics, operative skills, outcomes, satisfaction and complications of liposuction in the posterior area of the neck.Methods:32 patients who suffered from the enlarged cervicodorsal fat pads were enrolled in this retrospective study in the 15th Department of Plastic Surgery Hospital,CAMS,PUMC.From November 2006 to August 2019,liposuction was performed to all patients under local anesthesia or combined with intravenous anesthesia. The clinical outcomes were recorded by plastic surgeons during the follow-up periods, including the pre-operative and post-operative photographs, patient self-assessments,observer assessments, recurrence rate and complications of the liposuction. Results:The average volume of liposuction in the nape of neck was 213.91ml.All the patients who underwent suction-assisted liposuction(32 cases)had completed the revisits with no recurrence. The follow-up time ranged from 1 month to 7.75 years and no severe complication was encountered. The wound healed primarily in 32 cases(1 00%)and the reoperation rate was 0%.Only one patient complained about numb, unignorable pain, and skin itching. The discomfort disappeared approximately one week after liposuction.Another patient had ecchymosis in the cervicodorsal area, which subsided after 2 weeks. Two patients had scars under the collar line with a length of 2.0~2.5mm,which were quite concealed. On the follow-up visits,13 cases(40.63%)were very satisfied with the clinical outcome(described as "Excellent"),the assessments from 12 patients(37.50%)were"Good" and 7 patients(21.88%)thought the outcome was "Fair".The assessment scores evaluated by the patients and plastic surgeons were 3.19±0.78(mean±SD)and 3.34±0.65(mean±SD).Conclusion:Suction-assisted liposuction of the cervicodorsal fat pad has been proved to be an effective treatment with hidden incisions, reduced injury and excellent outcomes, followed by the application of pressure dressing. The sharp-tip cannulas(2.5-mm and 3.0-mm)could be well utilized to remove the enlarged subcutaneous adipose tissue with compact connective tissue in the nape of neck. The treatment of cervicodorsal fat pads based on liposuction is a safe, effective and highly acceptable option with an outstanding outcome, which is worth applying and spreading widely.
