Background: Acute pancreatitis(AP)is one of the common critical diseases of gastrointestinal diseases that cause abdominal pain.The incidence of acute pancreatitis is increasing year by year in China.It is one of the most common gastrointestinal diseases in hospitals in China that cause poor patient prognosis and even death.People have made substantial progress in understanding the pathophysiology of acute pancreatitis,and metabolic dysregulation and energy metabolism disorders play a key role in the development of acute pancreatitis.At present,the main treatment is symptomatic treatment,and there is a lack of effective specific nutrition with Supporting treatment.This topic studies the role of glucose metabolism and amino acid metabolism in acute pancreatitis through broad targeted metabolomics,and provides a more powerful evidence for clinically seeking nutritional targets for the treatment of acute pancreatitis,thereby improving clinical prognosis and outcome of acute pancreatitis.Methods: We quantified the serum metabolite concentration of acute pancreatitis based on broad targeted metabolomics,and compared it with acute cholecystitis and healthy control to analyze variation of energy metabolism in patients with acute pancreatitis.This experiment uses derivatization of metabolites in serum,and uses gas chromatography coupled to mass spectrometry(GC-MS)to qualitatively and quantitatively analyze each metabolite in plasma.peak area by internal standard correction will be performed with unit and multivariate statistical analysis.Results: Data analysis found that there are 58 metabolites that are different from acute pancreatitis compared with acute cholecystitis and healthy controls.Based on significantly different metabolites,Metabo Analyst 4.0 was used to analyze the dysregulation metabolic pathways.It was found that the following metabolic pathways were involved:(p <0.05,ipact>0.01): Alanine,aspartic acid and glutamic acid metabolism;tricarboxylic acid cycle(TCA cycle);glutamine metabolism;ascorbic acid and uronic acid metabolism;arginine synthesis;starch and sucrose metabolism;Cysteine and methionine metabolism.Among them,the down-regulation of the intermediates of fumaric acid and succinic acid in the TCA cycle has suggested mitochondrial energy metabolism disorders;While the rise in amino acids suggests increase of gluconeogenesis,which is used to compensate the body for the high energy load of acute pancreatitis,leading to a general uptrend in breakdown of protein in the body and a negative nitrogen balance.Conclusion: The TCA cycle of patients with acute pancreatitis is dysregulation,and amino acids continuously enter the TCA cycle through gluconeogenesis to compensate for energy supply.Research on metabolomics of acute pancreatitis provided strong data support for clinical analysis and detection.Through the analysis of dysregulation of metabolic pathways,the effects of differences of metabolite markers,on the oxidative stress process of acute pancreatitis were explained.influences.Among them,the intermediates of TCA cycle(fumaric acid and succinic acid)are promising targets for clinical treatment of acute pancreatitis.Therefore,this study of t energy metabolism mechanism research provides scientific and useful guidance value clinical treatment of acute pancreatitis.
