Objective To explore the distribution characteristics of TCM constitutions of the elderly and their influencing factors,so as to provide a basis for the early prevention and treatment of TCM constitutions of hypertension and diabetes, and to explore the improvement effects of intervention measures for the health status based on TCM constitution identification through intervention trials,in order to provide a basis for improving the health of the elderly. Methods The elderly in Cangshan District of Fuzhou City were selected as the research objects to analyze the distribution characteristics of TCM constitution by a cross-sectional study;the relationship between TCM constitutions, constitutional elements and hypertension, diabetes,as well as the influencing factors of constitution elements related to the said two diseases were investigated by logistic regression analysis;Intervention experiment: the control group was given routine guidance, while the intervention group was given health education index based on their constitution type. The differences of SF-36 scores between the two groups before and after the intervention were analyzed by repeated measures analysis of variance and analysis of covariance.Results(1)The TCM constitutions of the elderly in the community were mainly phlegm-dampness constitution(34.9%)and yin deficiency(26.1%).There were statistically differences in physical distribution among different genders, ages, hypertension, diabetes, smoking, drinking, drinking tea,BMI and physical exercise.(all P<0.05).(2)Analysis of logistic regression results demonstrated that the deficiency of yin deficiency(P=0.016)and phlegm-dampness constitution(P<0.01)increased the risk of hypertension, with OR values of 1.715 and 2.534,respectively;those with yin deficiency(P=0.028)and wet phlegm(P=0.030)had an increased risk of diabetes, with OR values of 1.883 and 1.847,respectively.(3)The elements of Phlegm-dampness Constitution(OR=1.446,P=0.005)may be the risk factors of hypertension in the elderly. The elements of Yin-deficiency Constitution(OR=1.384,P=0.026)may be the risk factors of diabetes in the elderly.(4)Further study the influencing factors of yin deficiency element and phlegm wetness element, the results showed that gender, average sleep quality, low sleep quality and time of each exercise may be the influencing factors of yin deficiency element, the OR values were 1.820 and 1.655,2.647,0.992 respectively.Smoking, average sleep quality, and overweight, obesity and time of each exercise may be the factors affecting phlegm wet quality element,OR values were 2.303,1.533,5.584,17.120,0.993.(5)The results of intervention covariance analysis showed that the scores of role physical, role emotional, social functioning, bodily pain, vitality and other dimensions after intervention were statistically significant between the two groups after controlling the influence of the scores before intervention(P<0.05).After controlling for the influence of the four dimensions of role physical, role emotional, social functioning, bodily pain, the score of the intervention group was higher than that of the control group, and the score of vitality in the intervention group decreased. Conclusion The elderly population in cangshan district, fuzhou city is predominantly biased, with phlegm-dampness constitution and yin deficiency being more common;gender, age, hypertension, diabetes, smoking, drinking, tea,BMI and physical activity all affect the TCM constitutions of the elderly;yin deficiency and phlegm-dampness constitution may be the risk factors for hypertension and diabetes in the elderly;in the merge-constitution,phlegm dampness element may be a risk of hypertension in the elderly, while yin deficiency element may be a risk of diabetes in the elderly. In addition, gender, average sleep quality, low sleep quality and time of each exercise may be the influencing factors of yin deficiency;smoking, average sleep quality, overweight, obesity and time of each exercise may be the influencing factors of phlegm-dampness constitution.Health interventions based on TCM constitution identification can improve the health status of the elderly.
