Objective:To explore the diagnostic value of low-dose spiral computed tomography for complex congenital heart disease(CHD).Methods:Retrospective analysis was performed on 40 cases of patients with complex congenital heart disease diagnosed by surgical treatment or cardiovascular angiography in our hospital from January 2011 to December 2016 who had complete clinical data and underwent preoperative cardiac examination using spiral CT and echocardiography.The radiation dose and diagnostic accuracy of patients examined by different CT machines were compared with the results of surgery or cardiovascular angiography as the gold standard.Then,the data of the two spiral CT machine scans were combined,and the diagnostic accuracy of spiral CT and echocardiogra m-phy was compared based on the results of surgery or cardiovascular angiography as the gold standard.Results:Surgery or angiography were found cardiovascular malformation of 147,which confirmed by surgery or angiography heart structural abnormalities 86,and 64-row spiral CT in the diagnosis of 38,dual-so urce CT in the diagnosis of 34,two kinds of spiral CT machine diagnosis 72,d-iagnostic accuracy rate of 83.72%,74 of echocardiographic diagnosis,diagnostic accuracy rate of 86.05%,echocardiography diagnosis accuracy is slightly higher than that of spiral CT,two types of inspection there was no statistically signif-icant difference comparing(χ2=0.18,P>0.05);Confirmed by surgery or angiography results abnormal heart outside the structure,a total of 61,and 64-row spiral CT in the diagnosis of 31,dual-source CT in the diagnosis of 30,two kinds of spiral CT machine diagnosis 61,diagnostic accuracy rate of 100%,echocardiography in the diagnosis of 40,diagnostic accuracy rate of 65.57%,compared with Yu Chaosheng cardiogram,spiral CT diagnostic accuracy improved obviously,comparing the results of the two types of inspection,there is statistical significance difference(χ2=25.37,P<0.01).In the two spiral CT machine examinations,a total of 69 cardiovascular malformations were detected by 64-slice spiral CT,with a diagnostic accuracy of about 90.79%.The dose length product(DLP)w-as 568.72±443.07 m Gy,and the effective radiation dose was 10.86±7.08 m Sv.A total of 64 cardiovascular malformations were detected by dual-source CT,and the diagnostic accuracy was about 90.14%.The dose length product(DLP)was85.8±58.59 m Gy,and the effective radiation dose was 1.92±0.94 m Sv.The diagnost ic accuracy of 64-slice spiral CT and dual-source CT was similar,but the effect-tive radiation dose of dual-source CT was significantly reduced.Conclusion:For the diagnosis accuracy of complex congenital heart disease with external structure malformations,spiral CT is higher than echocardiography,and the results are statistically significant,and there is no significant difference between the accuracy rate of diagnosing intracardiac malformations and that of echocardiogramphy,Considering that echocardiography is non-invasive and costs less,the combination of the two is more conducive to the diagnosis of complex congenital heart disease.There was no significant difference in the accuracy of 64-slice spiral CT and dual-source CT in diagnosing cardiovascular structural malformations of complex congenital heart disease,but dual-source CT visibly reduced the radiation dose of the examination.
