| The report of the 19 th National Congress of the Communist Party of China points out that at present,the major social contradictions in China have been transformed into the contradiction between the people’s growing demand for a better life and the unbalanced and inadequate development.With the development of China’s economy and society,people in the new era have higher requirements for health.However,the current situation of medical treatment in China is that there are few patients in small hospitals at the grass-roots level.However,there are many patients in large hospitals,and the phenomenon of overcrowding is still common.Promoting the hierarchical diagnosis and treatment system is an important measure to improve the difficulty and cost of seeing a doctor.In 2015,the State Council issued the guiding opinions on promoting the construction of the hierarchical diagnosis and treatment system,and began to promote the construction of the hierarchical diagnosis and treatment system in China.At the end of 2015,Chongqing issued the implementation opinions on carrying out the pilot of hierarchical diagnosis and treatment system,and began to establish a hierarchical diagnosis and treatment system.Mainly from the system construction,general practitioner training,the establishment of medical security system,improve the overall level of grass-roots medical institutions and other comprehensive promotion of hierarchical diagnosis and treatment.Chongqing has made some achievements in the implementation of hierarchical diagnosis and treatment,but there are also some problems,such as difficult to see a doctor.What is the root of the problem of "difficult to see a doctor" ? What are the factors restricting the implementation of classified diagnosis and treatment in Chongqing? How should relevant departments promote classified diagnosis and treatment in Chongqing? This study is based on the analysis of domestic and foreign hierarchical diagnosis and treatment research,with the help of literature research and questionnaire survey,to analyze the status quo,existing problems and causes of hierarchical diagnosis and treatment in Chongqing,and to explore how to improve the hierarchical diagnosis and treatment system in Chongqing.The basic conclusions are as follows:First,the awareness rate of the general public in Chongqing to the hierarchical diagnosis and treatment is relatively low,and the awareness rate of the medical workers to the hierarchical diagnosis and treatment is relatively high,while the way for the medical staff to understand the hierarchical diagnosis and treatment is mainly through working contact.However,the proportion of TV,newspaper and other media is relatively low,which shows that the promotion of hierarchical diagnosis and treatment among ordinary people needs to be strengthened.Second,Chongqing residents are more willing to go to a large hospital for the first visit after they are ill.Due to the high level of medical care in large hospitals,advanced medical equipment,guaranteed medical insurance reimbursement,complete drugs and timely use of drugs,the public feel that large hospitals are more reliable.Thirdly,the recognition of Chongqing citizens to primary medical institutions is not high.It is mainly considered that the basic medical institutions have low medical care level,backward medical equipment,low medical insurance reimbursement,incomplete drugs and poor health environment.The number of basic medical and health institutions per capita in Chongqing shows a downward trend.According to the City Health Statistics Yearbook,the number of community health service centers(stations)and health centers of primary medical institutions in Chongqing is on the decline,while the total population of Chongqing is on the rise,and the number of primary medical institutions per capita is on the decline,and the citizens can go to the primary medical institutions further and more inconveniently.Fifth,people still find it difficult to see a doctor when they go to a large hospital.The main factors that make it difficult for residents to see a doctor are the large number of people queuing for registration,the long waiting time,the complicated reimbursement procedures of medical insurance,and the tension of hospital beds.After the implementation of the hierarchical diagnosis and treatment system in Chongqing,the problem of "difficult to see a doctor" has not been solved effectively. |
