Objective: Explore the application of the accelerated rehabilitation surgery(ERAS)concept in artificial hip replacement for the treatment of elderly femoral neck fractures,analyze the effect of the ERAS concept on early recovery and prognosis of patients,and the value of its promotion and application.Methods: Taking the time point of launching ERAS in the Department of Traumatology and Orthopedics of the First Affiliated Hospital of Guangxi Medical University as a boundary,we retrospectively analyzed the two years before application of ERAS(2016.01.01-2017.12.31)and the two years after application(2018.01.01-2019.12.31)64 elderly femoral neck fracture patients admitted to this department;among them,30 patients before ERAS were used as the control group,and 34 patients after ERAS were used as the ERAS group;the control group used traditional intervention methods during the perioperative period,ERAS group During the perioperative period,the concept of ERAS was used for intervention.Analyze and compare the patient’s pain VAS scores on the first and second days after surgery,time to ground,postoperative complications,perioperative and postoperative hospital stays,hospitalization costs,and hip joints at 1 and 3 months after surgery Function Harris score.RESULTS: 1.There were no statistically significant differences in the gender,age,weight,cause of injury,anesthesia ASA grade,and surgical procedures of patients in the control group and ERAS group(P>0.05),and there was comparability between the groups.2.There was no statistically significant difference in the pain VAS scores of the patients in the control group and the ERAS group on admission.The VAS scores of the patients in the ERAS group on the first and second days after surgery were significantly lower than those in the control group(P<0.05).The control group went to the ground earlier(P<0.05).3.During the perioperative period,10 patients(33.3%)in the control group had complications,and 4 patients(11.7%)in the ERAS group had complications.The incidence of complications in the ERAS group decreased significantly(P<0.05).4.The Harris score of hip joint function in the ERAS group was significantly higher than that in the control group at the first and third months after operation(P<0.05).5.Compared with the control group,the ERAS group reduced the waiting time by 1.04 days,the postoperative hospital stay decreased by 2.49 days,the total hospital stay was shortened by 3.53 days,and the hospitalization fee was reduced by 0.55 million yuan.The differences were statistically significant(P<0.05).CONCLUSION: After applying the ERAS concept in hip arthroplasty surgery for elderly femoral neck fractures,the patient has less pain,early time to go to the ground,fewer complications,faster hip function recovery,and shorter hospital stay;compared with the traditional concept,the effect is better,the patient benefits significantly,it is worth Promotion and application. |
