Backround The discovery and diagnosis of pancreatic cystic lesions are becoming more and more.Contrast-enhanced endoscopic ultrasonography play an important role in the diagnosis of cystic lesions of the pancreas.We aimed to explore the differences of different types of pancreatic cystic lesions under contrast-enhanced endoscopic ultrasonographyMethods: Patients who underwent contrast-enhanced endoscopic ultrasonography for evaluation of the intracapsular lesions and the cystic wall were included.The enhancement intensity、enhancement pattern、enhancement mode and time-intensity of the enhancement in the intracapsular and cystic wall of different types of pancreatic cystic lesions were compared.Result: A total of 30 cases were included from July 2014 to April 2020.There were 22 cases of tumorous lesions,including 4 cases of benign lesions,9cases of borderline lesions,9 cases of malignant lesions,and 8 cases of non-tumor lesions.Under contrast agent development,tumorous lesions were mostly enhanced,and non-tumor lesions were mostly not enhanced,both of which were different in the intracapsular and the cystic wall(P <0.05).In tumorous lesions,the intensity of enhancement: In the cyst wall,the benign is hyperenhancement,and the malignant is mostly hypoenhancement.There is a significant difference between the two(P <0.05).The borderline lesions can be isoenhancement,isoenhancement,and hypoenhancement,no difference between the borderline and the benign,the borderline and malignant.There was no significant difference in benign,borderline,and malignant in the intracapsular.Enhancement pattern: Benign lesions is rod-like,and malignancy is mostly dot-like enhancement,both of which are different in the intracapsular and the wall part(P <0.05).The borderline can be enhanced with dots and rods,and there is no difference between the borderline and the benign,the borderline and malignant in the intracapsular and the cystic wall.The enhancement mode and time-intensity of the enhancement : Benign,borderline,malignant in the cyst and the cyst wall enhancement phase overlap,no statistical difference between the three.Conclusions: Contrast-enhanced endoscopic ultrasonography can clearly show the boundary of the lesion and the internal structure of the cyst.At the same time,combining the enhanced intensity、enhancement pattern、enhanced mode and time-intensity of enhancement can help distinguish different types of pancreatic cystic lesions.
