To Analysis Of Thoracoscopic Decortication And Thoracotomy In The Treatment Of Stage Ⅱ Empyema

Objective:To compare and analyze the therapeutic effect of thoracoscopic pleural fiberboard peeling and thoracotomy in the treatment of stage Ⅱ empyema,and to provide reference for clinical treatment of stage Ⅱ empyema.Methods:To provide reference for clinical treatment of stage Ⅱ empyema,61 patients with stage Ⅱ empyema admitted to the thoracic surgery department of Jilin Medical College and the Affiliated Hospital of Yanbian University from October 2016 to March 2020 were divided into two groups:the thoracoscopic group(n=31)and the thoracotomy group(n=30),The clinical data,operation time,intraoperative bleeding volume,postoperative pain days,VAS pain score,postoperative chest drainage days and postoperative average hospitalization days were compared between the two groups.Results:1.Operation time:165.16±9.50min in thoracoscopic group and 150.70±8.46min in thoracotomy group,there was no statistical difference between the two groups(P>0.05)2.Intraoperative bleeding volume:209,30±13.40ml in the thoracoscopic group and 347.61±16.60ml in the thoracotomy group,with statistical difference between the two groups(P<0.05).3.Postoperative drainage days:5.23±1.12 days in the thoracoscopic group and 7.7 ± 1.69 days in the thoracotomy group,there was statistical difference between the two groups(P<0.05).4.Postoperative hospitalization days:9.2±1.42 days in the thoracoscopic group and 12.4±2.39 days in the thoracotomy group,with statistical difference between the two groups(P<0.05).5.Postoperative pain days:3.48±0.98 days in the thoracoscopic group and 5.67±1.17 days in the thoracotomy group,there was statistical difference between the two groups(P<0.05)6.VAS pain score:3.17±0.79 days in thoracoscopic group and 5.71±1.31days in thoracotomy group.There was statistical difference between the two groups(P<0.05).Conclusion:1.Thoracoscopic removal of pleural fiberboard in stage Ⅱ empyema has the advantages of small incision and good integrity of chest wall.2.Compared with thoracotomy,the volume of bleeding,the days of hospitalization,the days of drainage,the days of postoperative pain and VAS pain score in the operation of stageⅡ empyema with thoracoscopy have obvious advantages.
