Objective:Explore EST(duodenal papilla dissection)in treatment of acute gallstone pancreatitis(ABP).The influence of patients with clinical manifestations and biochemical indexes.Contrast ABP preoperative patients with postoperative intestinal adhesive COX-2mRNA gene expression,and EST influence for severe acute pancreatitis(SAP)with mild acute pancreatitis(MAP)in patients with COX-2mRNA expression and correlation.Method:Observed during June 2015 to August 2015,20 cases of hospitalized patients with diagnosis of ABP,women15 cases,15 cases were male,range from 38 to 80 years in age,62.0 ± 14.0 years on average.According to the presence of sustainability Officer function failure(for more than 48 hours),or local complications,the Ranson score,MCTSI scoring system divided into two group,SAP and MAP.Extraction of two groups of EST preoperative patients with postoperative intestinal mucosal tissue and blood,should be quantitative RT-PCR technique is used to inspect the twelve field mucosa COX-2 mRNA expression.Results:20 cases of ABP,the EST after interventional treatment,postoperative 1 w eek more preoperative biochemical indicators improved significantly(P<0.01);Two gro ups of 20 patients with ABP,preoperative duodenal mucosa COX-2mRNA express SAP group was obviously higher than MAP(mild group:0.75±0.091,severe group:2.71±0.149)(P<0.05),statistically significant;Two groups of 20 patients with ABP,the EST in interve ntional treatment of postoperative 1 week prior COX-2mRNA expression significantly r educed(mild group:0.35±0.069,severe groups:1.37±0.088)(P<0.05),with statistical signi ficance.Conclusion:ABP after EST intervention in patients with clinical manifestations and biochemical index improved significantly.In patients with ABP severely affected relatively mild intestinal mucosa COX-2 mRNA expression is high,the EST interventional treatment of postoperative intestinal mucosa COX-2 mRNA protein expression was decreased.
