Objective:To observe the changes of miR-21-5p and inflammatory cytokines in peripheral blood mononuclear cells of metabolically healthy obese adolescents and metabolically abnormal obese adolescents before and after intervention,and to further analyze the correlation between miR-21-5p changes and inflammatory cytokines.Methods:Eighteen obese male adolescents(aged 12-18 years)from Haoqian Weight Loss Training Camp were selected as subjects,with BMI reaching the obesity standard of the same age group.According to the screening criteria of metabolic syndrome,they were divided into metabolically healthy obese group and metabolically abnormal obese group.At the same time,8 metabolically healthy adolescents of the same age group with normal weight were selected as the control group at the early stage.Subjects in obese adolescents participated in a 4-week intervention of exercise and diet to reduce fat and weight.The glucose and lipid metabolism indexes of the subjects were measured before intervention,including:high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),insulin,blood glucose,triglyceride(TG),total cholesterol(TC).Body morphology indexes(body weight,BMI,body fat mass,fat percentage,adipode-free soft weight,waist circumference,hip circumference,abdominal circumference,waist-hip ratio,etc.),miR-21-5p expression level in peripheral blood mononuclear cells(real-time fluorescence quantitative PCR)and serum inflammatory factors(IL-17A,NF-?B p65,MCP-1,Hs CRP,ELISA method)in obese adolescents before and after intervention and in the control group were measured.Statistical software SPSS 23.0 was used for independent sample t-test,paired sample t-test and correlation analysis.All indicators were described as mean±standard deviation(P<0.05 indicated significant difference,P<0.01indicated extremely significant difference).Results:1.For body weight(96.35±11.78kg,103.41±25.77kg vs.62.51±5.56kg),BMI(33.13±2.77kg/m~2,34.11±5.65kg/m~2 vs.21.64±1.08kg/m~2),fat percentage(33.12±2.93%,33.49±4.11%vs.15.14±2.38%),waist circumference(103.45±8.43 cm,107.91±14.84 cm vs.71.89±3.58 cm),hip circumference(112.63±6.64 cm,114.98±11.52 cm vs.91.15±3.13 cm),waist-to-hip ratio(0.92±0.04,0.94±0.04 vs.0.79±0.03).Metabolically healthy obesity group and metabolic abnormality group were significantly higher than normal weight group(P<0.01).There was no significant difference between metabolically abnormal obesity group and metabolically healthy obesity group(P>0.05).For metabolic indicators,Total cholesterol(4.63±0.87 mmol/L,5.43±0.80 mmol/L vs.3.69±0.69 mmol/L),low density lipoprotein(LDL)cholesterol(2.88±0.61 mmol/L,3.37±0.70 mmol/L vs.1.97±0.59 mmol/L),triglycerides(1.29±0.33 mmol/L,2.82±0.75 mmol/L vs.0.93±0.42 mmol/L),insulin(169.82±48.19 pmol/L,314.54±125.13 pmol/L vs.74.49±22.03 pmol/L)and insulin resistance index(4.91±1.97,8.29±3.04 vs.2.25±0.70)in the metabolically healthy obese group and the metabolically abnormal obese group were significantly higher than those in normal body weight group(P<0.01 or P<0.05).The total cholesterol(5.43±0.80 mmol/L vs.4.63±0.87 mmol/L),triglyceride(2.82±0.75 mmol/L vs.1.29±0.33 mmol/L),insulin(314.54±125.13 pmol/L vs.169.82±48.19 pmol/L)and insulin resistance index(8.29±3.04 vs.4.91±1.97)of the metabolically abnormal obese adolescents were significantly higher than those in metabolically healthy obese group(P<0.05).2.Compared with before the intervention,body weight,BMI,body fat mass,fat percentage,adipode-free soft body weight,waist circumference,hip circumference,abdominal circumference and waist-hip ratio of obese adolescents were significantly decreased after intervention(P<0.05).3.Compared with before the intervention,miR-21-5p in metabolically healthy obese group was not significantly changed after intervention(3.93±1.43 vs.3.08±1.25),but significantly decreased in metabolically abnormal obese group(3.15±1.05 vs.1.32±0.62)(P<0.05).4.Compared with before the intervention,the inflammatory factors of obese adolescents changed significantly after the intervention.NF-?B p65(0.25±0.17 ng/m L vs.0.19±0.14 ng/m L)and hs CRP(4.45±4.27 mg/L vs.1.56±1.16 mg/L)were significantly decreased in metabolically healthy group(P<0.05),but IL-17A and MCP-1 had no significant changes(P>0.05).For the metabolically abnormal group,IL-17A(43.21±18.83 pg/m L vs.35.38±14.86 pg/m L),MCP-1(265.46±136.30 pg/m L vs.211.38±99.19 pg/m L)and hs CRP(3.41±2.42 mg/L vs.1.53±0.93mg/L)were significantly decreased(P<0.05),but there was no significant change in NF-?B p65(P>0.05).5.Correlation studies showed that there was no significant correlation between miR-21-5p and various inflammatory factors,but there was a very significant correlation between inflammatory factor MCP-1 and IL-17A(P<0.01).Conclusions:1.Four weeks of enclosed exercise combined with dietary intervention can significantly reduce the weight,circumference,body fat amount and fat percentage of obese male adolescents,improve their body shape and body composition,and have a good weight loss effect.2.Four weeks of enclosed exercise combined with dietary intervention can improve the inflammatory state of the body of obese male adolescents.3.Four weeks of enclosed exercise combined with dietary intervention can effectively reduce the expression level of miR-21-5p in peripheral blood mononuclear cells in metabolically obese male adolescents.
