The purpose of this project was to develop a grant to fund an intense case management program to address the challenges of community-dwelling frail disabled older adults 60 years and older residing in Los Angeles County. These challenges include having disabilities, co-morbid medical conditions, and life stressors that affect their cognitive and physical functioning. These challenges can also affect their ability to continue to live independently and can compromise their quality of life. The California Wellness Foundation was chosen as the foundation most appropriate for possible funding. A grant was written to seek funding from The California Wellness Foundation in support of an intense case management program to serve frail disabled older adults to continue to live independently and to maintain their quality of life.
