Clinical Observation On Xingqi Qingjiang Self-prepared Decoction In Treating Gastroesophageal Reflux Disease With Gallbladder Heat And Stomach

Objective:Gastroesophageal Reflux Disease(GERD)is a common chronic recurrent disease of the digestive system.The incidence rate of GERD in China is increasing year by year,and has become one of the focuses of domestic scholars.Therefore,the research on GERD prevention and treatment has important social practical significance.In this study,through observing the clinical effect of xingqiqingjiang self-made formula in the treatment of GERD of bile heat invading the stomach,and verifying the therapeutic effect of the method of axis and wheel parallel movement on GERD of bile heat invading the stomach,we can provide a new dialectical thinking and treatment scheme for GERD of bile heat invading the stomach with traditional Chinese medicine.Methods:This study was carried out in the digestive department of Dongzhimen Hospital.we using a prospective randomized controlled design.72 patients with GERD of gallbladder heat invading stomach type were included in the study,who signed the informed consent.They were randomly divided into the Treatment group and the control group with 36 cases in each group.The Treatment group was treated by oral administration of xingqiqingjiang self-made formula(usage:twice a day,taken one bag half an hour before breakfast and one bag two hours after supper),the control group was treated by pantoprazole sodium enteric coated capsule(usage:taking once a day,40mg each time,half an hour before breakfast),mosapride citrate tablets(usage:3 times a day,5mg each time,taken before three meals),and the two groups were treated by oral administration.The course of treatment was 8 weeks.Professionals recorded general information,Gerd-Q scale before and after treatment,TCM Syndrome Scale and adverse reactions during treatment.The data of above were input into Excel,and SPSS 25.0 statistical software was used to analyze the data to evaluate the clinical efficacy.Results:72 cases of GERD were included in this study.There were 8 cases falling off during the treatment and 64 cases actually completed,including 34 cases in the Treatment group and 30 cases in the control group.(1)general data:in 64 patients,the proportion of male and female is 1:1.37,women are slightly more than men.in this study,the age is defined in 18-60 years old,including 64 patients,the age distribution is 23-57 years old,of which 40-49 years is the peak.There was no significant difference between the two groups in gender,age and course of disease(P>0.05).The predisposing factors were diet(37.50%)and emotion(20.31%).In this study,25 people were addicted to cigarette and alcohol,accounting for 39.06%.In terms of food preference,those who were partial to meat,fried,smoked and roasted(18.75%),those who liked to eat too spicy,too cold,too hot and too sweet(14.06%),and those who liked to drink coffee(10.94%).(2)efficacy index:in terms of Gerd-Q score,after 8 weeks of treatment,the Gerd-Q score of the Treatment group and the control group decreased compared with that before treatment,with statistical significance(P<0.05),reflecting the improvement of Gerd-Q score in both groups,with no statistically significant difference between the two groups(P>0.05).In the aspect of TCM syndrome score,after 8 weeks of treatment,the total score of TCM syndrome in the Treatment group and the control group decreased significantly compared with that before treatment(P<0.05),and the total score of TCM syndrome in the Treatment group decreased significantly compared with that in the control group(P<0.05),reflecting that the Treatment group is better than the control group in improving the total score of TCM syndrome,and better in the symptoms of bitter taste,dry throat,chest and flank pain,irritation,insomnia and dreaminess(P<0.05),but there was no significant difference between the two groups in improving the symptoms of heartburn,chest and back pain,acid regurgitation,belching or food regurgitation and starvation(P>0.05).In the evaluation of TCM syndrome efficacy,there were 34 cases in the Treatment group,8 cases cured,16 cases obviously effective,7 cases effective and 3 cases ineffective.The effective rate of the Treatment group was 91.18%.30 cases in the control group,5 cases cured,4 cases obviously effective,18 cases effective and 3 cases ineffective.The effective rate of the control group was 90.00%.There was no statistical significance between the two groups(P>0.05).(3)no obvious adverse reactions occurred in the two groups during the treatment.Conclusion:the clinical effect of xingqiqingjiangzi formula on GERD of gallbladder heat invading stomach type is exact,which is equivalent to that of pantoprazole sodium enteric coated capsule combined with mosapride citrate,but it is better than pantoprazole sodium enteric coated capsule combined with mosapride citrate in improving the symptoms of bitter taste,dry throat,chest and flank pain,irritation,insomnia and dreaminess.There was no obvious adverse reaction.It is suggested that the treatment of GERD of gallbladder heat invading stomach by using the method of combined axial and circular circulation is worthy of clinical application.
