Observation On The Curative Effect Of Acupuncture In The Treatment Of Cognitive Dysfunction After Orthopedic Surgery For The Elderly Under General Anesthesia

Purpose:To observe and evaluate the short-term therapeutic effect of acupuncture on the elderly patients with cognitive impairment after lower extremity operation under general anesthesia.Material and method:From May 2018 to December 2019,60 patients with cognitive impairment within 48 hours after extubation were randomly divided into control group,scalp acupuncture group and scalp + body acupuncture group The acupuncture treatment group was divided into three groups,each group was divided into 20 cases.The control group was given 100 ml of 0.5% sodium chloride solution plus 2G of olacetam for injection once a day to improve the nutrition of brain nerves;the scalp acupuncture treatment group was given Baihui,Sishencong,Yintang and shenting on the basis of nutritional neurodrugs in the control group,and the acupuncture was kept for 30 min once a day;the scalp acupuncture + body acupuncture treatment group was given nutritional neurodrugs in the control group After acupuncture at Baihui,Sishencong,Yintang,shenting,Neiguan,Shenmen,Zusanli and Sanyinjiao(the latter four acupoints are all bilateral acupoints)for 30 minutes,once a day;after acupuncture for 7 days,observe the MMSE score and NSE value in serum of patients on the day,the third day and the seventh day.Results: 1.MMSE score: on the first day of acupuncture,there was no significant difference in MMSE scores among the three groups(p>0.05);After 3 days of acupuncture,the MMSE scores of scalp acupuncture+body acupuncture group and scalp acupuncture group were significantly improved compared with the control group(p<0.05),while the MMSE scores of scalp acupuncture+body acupuncture group and scalp acupuncture group had no statistical difference(p>0.05).After acupuncture for 7 days,the MMSE score in scalp acupuncture+body acupuncture group was improved more obviously than that in control group and scalp acupuncture group(p<0.05).2.NSE value: on the first day of acupuncture,there was no statistical difference in NSE values among the three groups(p>0.05);After 3 days of acupuncture,the NSE values in the scalp acupuncture+body acupuncture group and scalp acupuncture group were significantly improved than those in the control group(p<0.05),while the NSE values in the scalp acupuncture+body acupuncture group and scalp acupuncture group had no statistical difference(p>0.05).After acupuncture for 7 days,the NSE value in scalp acupuncture+body acupuncture group was improved more obviously than that in control group and scalp acupuncture group(p<0.05).Conclusion:Acupuncture at Baihui,Sishencong,Yintang,shenting,Neiguan,Shenmen,Zusanli and Sanyinjiao(two sides of the last four acupoints)can significantly improve postoperative cognitive dysfunction,improve MMSE score,reduce NSE value in serum,speed up the recovery of patients,correspondingly shorten the postoperative recovery time of patients,with more significant effect.
