Purpose: To evaluate the clinical efficacy of integrated Chinese and western medicine in treating mycoplasma pneumoniae pneumonia in children through the improvement of symptoms after treatment compared with before treatment and metabolic analysis.Material and method:In this study,all the patients were selected to visit the pediatric ward of the affiliated hospital of liaoning university of traditional Chinese medicine in September,2018 and march,2019,and a total of 11 cases of mycoplasma pneumoniae pneumonia(phlegm-heat type)were confirmed and admitted.A total of 11 normal children were selected from the physical examination center of children affiliated hospital of liaoning university of traditional Chinese medicine at the same time.The score of main symptoms and signs(fever,cough,sputum,asthma,lung auscultation),the score of secondary symptoms(spitting,mental irritability,red face,cyanosis,dry stools,yellow urine,dry nose,red lips),the total score of diseases and symptoms,and the rate of recovery before and after treatment were recorded and compared.The metabolic spectrum of mycoplasma pneumoniae pneumonia in the serum of children with mycoplasma pneumoniae pneumonia before and after treatment and normal children was established by using liquid/mass Analysis,and the matrix data were statistically analyzed by Principal Component Analysis(PCA)and Orthogonal Principal square Discriminant Analysis(OLPS-DA).Screening potential serum biomarkers and evaluating clinical efficacy.Results:1.Before treatment,gender,age,height,weight and other basic information of children in the two groups were compared,with P value greater than 0.05,which was comparable.2.After treatment,the score of main symptoms and signs,the score of secondary symptoms,the total score of symptoms and the single score of main symptoms and signs were significantly lower than before treatment(P < 0.05).The efficacy evaluation found that the obvious effect accounted for 90.90%,the effective effect accounted for 9.10%,and the total effective rate was 100%.3.After metabolomics PCA and OLPS-DA analysis,it was found that some metabolites were collected to normal children after treatment,and the trend of fusion appeared.After treatment,metabolites of normal children were more discrete than before treatment.4.In the positive ion mode,258 metabolites were significantly increased and 486 metabolites were decreased in TA group compared with CK group.There were 189 metabolites significantly increased in TB group and 418 metabolites in CK group.There were 327 metabolites significantly increased in TB group and 345 metabolites in TA group.In the anion mode,55 metabolites were significantly increased and 223 metabolites were decreased in TA group compared with CK group.Compared with CK group,131 metabolites and 102 metabolites were significantly increased in TB group.Compared with TA group,200 metabolites in TB group were significantly increased and 42 metabolites were decreased.5.Through metabolomic analysis,15 kinds of metabolites were found to have trend differences between normal children and children before and after treatment..They were phosphatidylcholine:PC(16:0/0:0),PC(p-18:1(9Z)/0:0),Lyso PC(18:0),Lyso PC(20:4(5Z,8Z,11 Z,14Z));sphingomyelin:SM(d18:1 / band of z(13)),SM(d18:1 / z(15)),SM(d18:0/18:1 z(11));triglycerides:TG(14:0/15:0/20:5(5Z,8Z,11 Z,14Z,17Z));Palmitic acid,Oleic acid and so on Conclusion:1.Integrated treatment of traditional Chinese and western medicine can effectively treat mycoplasma pneumonia in children,relieve the main and secondary symptoms and signs of mycoplasma pneumonia,improve the overall condition of the disease,and improve the quality of life of children.2.PCA and OLPS-DA analysis found that metabolites tended to converge to the normal concentration after treatment and the dispersion between metabolites before and after treatment was relatively large,indicating that mycoplasma pneumoniae infection led to metabolic dysfunction in human body,and this treatment scheme could correct its metabolic function.3.Through metabolomic analysis,it was preliminarily found that mycoplasma pneumoniae pneumonia may lead to abnormal metabolism of glycerol and phospholipid,sphingolipid andtriglyceride,and cause lipid metabolism disorder in the body.
