| Purpose:The purpose of this study was to explore whether the combination of traditional Chinese medicine decoction gentiana macrophylla sinensis and traditional Chinese medicine acupuncture has more advantages than traditional western medicine in the clinical treatment of vestibular neuritis.Material and method:A total of 73 patients with vestibular neuritis admitted to the otorhinolaryngology unit of the affiliated hospital of liaoning university of traditional Chinese medicine from December 2017 to December 2019,who met the inclusion criteria and exclusion criteria, were randomly assigned to the treatment group of 37 patients and the control group of 36 patients. The control group was treated with conventional western medicine,i.e.,one intramuscular injection of fenarrhizone 25 mg per day for three days to relieve the patients’ vertigo symptoms.Methylprednisolone tablets should be taken once a day for 48 mg for 3 days, reduced to 40 mg after 3 days, taken once a day for 3 days, and so on,decreasing by 8mg to 0mg every 3 days.Intravenous infusion of 70 mg + 0.9% sodium chloride solution once a day;Oracetam injection 15ml+ 0.9% sodium chloride solution 100 ml intravenous drip once a day,and daily vestibular rehabilitation training. On the basis of the control group, the treatment group took 100 ml of gentiana macrophylla macrophylla decoction three times a day for 2 weeks.Acupuncture was performed at yitang point, shenting point, baihui point and the bilateral halo hearing area. The needle was inserted obliquely, followed by tonifying, tonifying, diarrhea and qi extraction. The needle was retained for 30 minutes, once a day for 2 weeks of continuous treatment.DHI scale filling, static balance test, video head pulse test, and subjective visual vertical line examination were performed at the time of treatment and 2 weeks after treatment, respectively.Spontaneous nystagmus detection was performed daily after treatment until spontaneous nystagmus disappeared. The efficacy was evaluated.Results:1.Patients in the treatment group had shorter static compensation time and more obvious dynamic compensation(P < 0.05).2.Compared with the control group, the effect of the treatment group on the improvement of patients’ static balance ability was more obvious(P<0.05).3.Both the treatment group and the control group had an improvement effect on the DHI scores of patients in each group(P<0.05).The treatment group was more effective than the control group in reducing DHI scores in each group(P<0.05),especially in reducing DHI-e(emotional score)and DHI total score(P<0.01).4.Both the treatment group and the control group could increase the regression gain value of the affected semicircular canal(P<0.05).5.The regression gain value of the affected semicircular canal was significantly increased in the treatment group compared with the control group(P<0.05).Conclusion:1.The treatment group could promote the static and dynamic compensation of patients with vestibular neuritis more quickly.2.The treatment group could better improve the vestibular dysfunction of patients.3.The improvement effect of the treatment group on emotional factors in patients with vertigo caused by vestibular neuritis was significantly better than that of the control group.4.The addition of gentiana macrophyllae decoction combined with acupuncture treatment will promote the compensation of vestibular function, high safety and little side effects. |
