| Purpose:This study summarizes the clinical research of acupuncture and moxibustion treatment of spastic hemiplegia after stroke in the past ten years,and classifies and summarizes the last acupoints according to their meridian distribution,location,and specific acupoints.Of acupuncture for the treatment of spastic hemiplegia after stroke and analysis of its compatibility.And evaluate the curative effect of acupuncture on spastic hemiplegia after stroke,and provide experience summary and evidence-based basis for clinical practice.Material flnd method:Using computer retrieval,we searched the clinical randomized controlled study of acupuncture and moxibustion for spastic hemiplegia after stroke published in the CNKI and VIP database for the past 10 years.Through systematic collation and screening,a total of 110 eligible documents were included.The acupoints,meridians,parts,and specific points selected in the literature are summarized and statistically obtained to obtain various data.The acupoints,meridians,parts,and specific points in the database are analyzed by frequency and frequency.The original data is standardized to form suitable data.The data form of the data analysis uses SPSS Statistics 25.0 statistical software,and uses factor analysis and cluster analysis methods to analyze and summariz e high-frequency acupoints larger than the average acupoints.SPSS Modeler 18.0 data mining software is used to associate the acupoints with the association rule algorithm.The relationship between meridian and its meridian compatibility was analyzed,and the acupuncture and moxibustion in modem literature for the treatment of spastic hemiplegia after stroke were summarized.Eleven clinical randomized controlled trials of clinical trials were screened from 110 literatures,and the M eta-analysis of randomized controlled trials that met the criteria was reviewed using Review Manager 5.3 software to evaluate the clinical efficacy of acupuncture treatment for spastic hemiplegia after stroke.Results:1.Of the 110 articles that met the standards,74 were acupuncture,E were electroacupuncture,6 were moxibustion,11 were warm acupuncture,10 were fire acupuncture,and 1 was pricking therapy.2.Statistical results:There were 151 acupoints selected,and the total frequency of acupoints used was 1239 times.There were 36 acupoints larger than the average acupoints,accounting for 23.68%of all acupoints and 74.98%of all frequencies.According to statistics,Hegu(61),Quchi(60),Sanyinjiao(55),Scapular(49),Yanglingquan(45),Wai gum(45),Ruler(45),Sea.of Blood(40).The cumulative frequency of the 15 acupoints of Yinlingquan(39).Shousanli(38),Neiguan(35),Zusanli(33),Jiquan(30),Zhaohai(27),and Taichong(25)exceeded Fifty percent(50.61%)have become a high-frequency acupuncture point for acupuncture and moxibustion in the treatment of spastic hemiplegia after stroke.The most frequently used meridians are the Hand Yangming large intestine meridian,followed by the Foot Taiyin spleen meridian.Foot Shaoyang bile meridian.Foot Sun bladder meridian,and Foot Yangming stomach meridian.The most frequently used points in a specific point are the five points for acupuncture and acupoints,followed by the acupoints,Xiahe and Bamai acupoints.According to the location distribution,the frequency of selecting acupoints in the lower limbs was 45.36%,the frequency of the upper limbs was 42.45%,and the frequency of the head and neck was 6.78%.3.Cluster analysis results:36 high-frequency acupoints can be divided into 5 clusters:①Sanyinjiao,Yinlingquan,Chize,Jiquan,Neiguan,Zhaohai,Daling,Xuehai,Liangqiu,Houxi,Quze:② Shenmai,Yangchi,Tongguan,Ququan,Patio,Jiexi,Fut;③ Yanglingquan,Zusanli,Hegu,Quchi,Shoulder,Waiguan,Hangzhong,Hand Sanli,arm cymbals,ring jump,Fengshi,Taichong,Qiu market;④ Weizhong,Chengshan,Yinmen,Baihui;⑤ Jiaji points.4.Factor analysis results:In the high-frequency acupoint factor analysis of acupuncture treatment of spastic hemiplegia after stroke,the group points outside the Guan,scapula,Quchi,hanging bell(absolute bone),Hegu,the highest factor contribution rate.5.Analysis results of association rules:Among the commonly used acupoints and acupoints in acupuncture for the treatment of stroke,Quchi-Hegu,Quchi-Waiguan,Quchi-shoulder,the three pairs have the highest compatibility frequency.There are 22 groups of acupoints with support>20%,and 2 groups of acupoints with 100%confidence:Quchi and Zusanli,Hegu;Sanyinjiao with Yinlingquan,and Ruler.There are 16 groups of meridian pairs with a degree of support>20%and a confidence of 100%.6.Evaluation of therapeutic effect of acupuncture on spastic hemiplegia after stroke:Acupuncture combined therapy is more effective than,rehabilitation or medication alone in treating spastic hemiplegia after stroke.In the Meta analysis of the four indicators of MAS,FMA,BI,and CSI,the difference between the treatment group and the control group was statistically significant.Among the meta analysis of FMA and BI,the heterogeneity was greater,and a sensitivity analysis was performed on this.It is found that the combined effects still have statistical significance after excluding each study.Conclusion:1.Acupuncture treatment of acupoints for spastic hemiplegia after apoplexy is mainly based on Yang Meridian points.,supplemented by Yin Meridian points.2.Specific acupoints are mainly dominated by specific acupoints,.of which Wuya and Jiaohui acupoints.3.The acupuncture and moxibustion treatment of spastic hemiplegic acupoints after apoplexy is mainly composed of Quchi,Zusanli and Hegu.4.The acupuncture treatment of spasmodic hemiplegia meridian is mainly based on the large intestine meridian of the hand Yangming and the small intestine meridian of the hand.5.Acupuncture combined therapy is more effective than rehabilitation or medication alone in treating spastic hemiplegia after stroke. |
