ObjectiveTo evaluate and analyze the effect of Zero-p on cervical spondylosis in ACDF and its influence on sagittal balance parameters of cervical spine,so as to provide guidance for future clinical treatment of cervical spondylosis.Methods74 patients in our group who underwent zero-p surgery from November 2016 to October 2018 were selected,including 28 males and 46 females,aged(49.36±9.87)years.VAS scores(Visual Analogue Score)and JOA scores(Japanese Orthopaedic Association Assessment and Treatment,Japanese Orthopaedic Association)were collected before,6 months after surgery and at the last follow-up.Association Scores,referred to as JOA,and sagittal balance parameters:C2-7Cobb angle,C2-7SVA(C2-7sagittal vertical axle base,C2-7-7 Sagittal Vertical Axis,referred to as C2-7SVA)and T1tilt angle,to evaluate the efficacy of zero-p in the treatment of cervical spondylosis from three aspects:symptom relief,functional recovery and sagittal balance reconstruction.ResultsAll the 74 patients completed the operation smoothly,the symptoms were alleviated to varying degrees,and the results were satisfactory.The patients were followed up for(18±6)months.The VAS score,JOA score,C2-7Cobb angle,C2-7SVA,T1 tilt angle changed from(7.56±0.81),(14.52±0.99),(8.60±10.84),(19.86±12.13)mm,(25.39±7.14)degree to the last follow-up,respectively.At the time of the interview,the scores were(0.09(±0.29),(16.69±0.57),(14.37±8.23),(18.91±10.47)mm,(28.02±6.49)degree.At 6 months after operation,the JOA score was significantly higher than that before operation(P<0.01).At the last follow-up,the JOA score of the patients was close to 17(full score).At 6 months after operation,the pain of the patients had been greatly relieved(P<0.01).By the last follow-up,the majority of the patients had no pain sensation;at 6 months after operation,the Cobb angle was obtained.At recovery(P<0.01),there was no significant difference between the last follow-up and half a year after operation(P>0.05);the change of SVA was not obvious after operation,and there was no statistical difference between the two groups;the T1 tilt angle increased at half a year after operation(P<0.01),and it was found that the tilt angle remained unchanged at the last follow-up.However,the angle of increase of tilt angle is not large,and has little effect on the overall structure of cervical spine.ConclusionZero-p has fewer complications and satisfactory results in the treatment of cervical spondylosis.The sagittal curvature of cervicalhas been improved significantly but the overall deviation has not been improved significantly.
