Background:many studies have shown that there may be some correlation between serum uric acid and hypertension,which is a risk factor of hypertension.However,the pathogenesis of hypertension is not completely clear,and some previous studies on the correlation between serum uric acid and hypertension did not adequately control the confounding variables.The purpose of this study was to investigate the correlation between serum uric acid and hypertension and the relationship between different levels of serum uric acid and the prevalence of hypertension.Methods:this study was a retrospective case-control study.We enrolled 1541 patients with hypertension hospitalized in the department of cardiology of Beijing hospital.After exclusion,a total of 365 patients with hypertension were included in this study,and 365 patients without hypertension hospitalized in the department of cardiology during the same period were matched as the control group.Multivariate logistic regression was used to investigate the relationship between serum uric acid and hypertension after adjusting for confounding variables.The study subjects were stratified according to uric acid level in blood,and the relationship between uric acid level in each layer and the prevalence of hypertension was analyzed.Two-sided P value less than 0.05 was statistically significant.Results:(1)baseline characteristics showed that the prevalence of hyperuricemia in the research group(30.4%)was higher than that in the control group(17.0%),and the blood uric acid level in the former group was 364.0(303.0,430.5)umol/1 higher than that in the latter group was 321.0(267.0,384.0)umol/1,with statistically significant differences(P<0.01).Multivariate logistic regression analysis was performed to adjust for other clinical covariates,and it was found that serum uric acid level was correlated with the occurrence of hypertension(OR=1.003,P<0.01).(2)In this study,the levels of uric acid in the blood were stratified as follows:Q1<300umol/1(n=232)、300umol/1≤Q2<360umol/1(n=191)、360umol/1≤Q3<420umol/l(n=145)、Q4≥420umol/1(n=162),the overall prevalence of hypertension in different serum uric acid was positively correlated,Ql=37.5%,Q2=49.20%,Q3=56.6%,Q4=63.0%.The differences were statistically significant(P<0.01).Compared with model 1,in Q1 groups for reference,model 2 on the basis of model 1 adjusting for the male,hyperuricemia,stroke,GLU and CRE,the prevalence of hypertension is still increasing,the OR were 1.644(95%CI:1.106~2.44.4,P=0.014),1.874(95%CI:1.190~2.951,P=0.007),1.718(95%CI:1.002~3.351,P=0.036),the differences were statistically significant(P<0.05).(3)the systolic blood pressure of the overall study population increased with the increase of serum uric acid level,showing an obvious linear relationship,and the difference was statistically significant(P<0.05).However,the linear relationship between diastolic blood pressure and uric acid level was not obvious,and there was no significant statistical difference.Conclusion:(1)there is a correlation between serum uric acid and hypertension,hyperuricemia may be a risk factor for hypertension.(2)the overall prevalence of hypertension increased with the increase of serum uric acid level.(3)There is a significant linear relationship between serum uric acid level and systolic blood pressure in the study population. |
