Objective:To discuss the academic thoughts of the professor Song Ping in the treatment of psoriasis.To determine whether there is abnormal metabolism in patients with psoriasis through cross-sectional investigation;To explore the possible biomarkers of abnormal metabolism in patients with psoriasis through metabolomics.Methods:1.To discuss the academic thoughts of Song Ping on the treatment of psoriasis in detail,and conduct a cross-sectional investigation in patients with psoriasis mainly about the metabolic characteristics.The contents included general information of 107 patients with psoriasis,metabolism-related clinical lab index,metabolism-related clinical symptoms,and metabolism-related lifestyle.The results of 67 healthy controls’ metabolic indexes were collected and were compared with the psoriasis patients.2.Small sample exploration was conducted based on UPLC-MS metabolomics technology,and the metabolic maps of 6 psoriasis patients and 6 healthy controls were compared,so as to tentatively explore the possible biomarkers of abnormal metabolism in psoriasis.Results:Research1:1.In 107 patients with psoriasis enrolled in this study,the mean BMI was(24.41±4.39),The abnormal rate of BMI was 50.94%,which was higher than that of the healthy control group,and the difference was statistically significant.With the increase of the PASI score,the BMI of the patients also increased,and the BMI value was positively correlated with the severity of psoriasis.With the increase of the disease course of psoriasis,the BMI of the patients also increased.2.In terms of laboratory indicators,most of the metabolic indicators in patients with psoriasis were abnormal compared with the healthy control group,mainly manifested as serum high TG,low HDL-C,and high UA,indicating abnormal lipid metabolism and purine metabolism in patients with psoriasis.3.In terms of metabolic system diseases,among 107 patients with psoriasis,a total of 44 patients were associated with metabolic system diseases,including hyperlipidemia,hypertension,fatty liver and diabetes,which appeared as one or a combination of metabolic diseases.Among them,the number of patients with hyperlipidemia was the largest,followed by hypertension,fatty liver and diabetes,the prevalence of metabolic diseases was higher than that of the healthy control group,but there was no statistical difference.4.In terms of lifestyle,patients with psoriasis prefer fat and greasy food.And the majority of patients do non-manual jobs,sitting on the chair for a long time,and do not like sports.Patients who like drinking and smoking are not a small number,and huge work pressure and irritable mood also performed in psoriasis patients.Research 2:Through UPLC-MS technology,there were 6 types of metabolites in patients with psoriasis,which namely PC(17:0/0:0),PC(0:0/18:1),PC(19:0/0:0),PC(0:0:20:2),CER(d18:0/h24:0),and CER(t18:0/24:0),showed a decreasing trend in patients with psoriasis.PC(17:0/0:0),PC(0:0/18:1),PC(19:0/0:0)and PC(0:0/20:2)are phosphatidylcholine(PC),which is glycerophospholipids.CER(dl8:0/h24:0)and CER(t18:0/24:0)are ceramic ide,which is sphingomyelin,indicating that phospholipid metabolism in patients with psoriasis was disturbed.Conclusion:Patients with psoriasis had overweight and obesity,abnormal levels of serum TG,HDL-C and UA,and the degree of metabolic abnormality is related to the severity and course of psoriasis.Psoriasis patients often associated with hypertension,diabetes,hyperlipidemia and other metabolic diseases.We should understand psoriasis from the perspective of holism,and consider that patients might be associated with metabolic syndrome,cardiovascular disease and other comorbidities.In the treatment strategies for psoriasis patients,we should also lower their metabolic abnormalities.In terms of TCM treatment scheme,eliminating dampness and phlegm might be effective.Through preliminary experiments,we found that the phospholipid metabolism in serum of psoriasis was out of order.Among them,the reduction of glycerophospholipid and sphingomyelin reflected the changes of their epidermal components and metabolism in vivo,and might be associated with the pathogenesis of psoriasis and comorbidity of the metabolic system.
