| Research backgroundLow back pain with an incidence of 80%is the most common disease in the world.It has many pathogenic factors and complicated pathological mechanism,and has always been the focus of medical research.Modern studies have found that,the abnormal changes in the function and structure of the polyfidus muscle,which maintain the stability of the lumbar spine,are closely related to low back pain.Polyfidus is a group of paravertebral muscles located in the innermost part of the deep spine and attached to the spinous process,lamina and transverse process.It is more developed in the waist,rich in muscle fibers,and thicker and shorter than other paravertebral muscles.In addition,the lumbar polyfidus muscle is the largest paravertebral attachment area and the only paravertebral muscle spanning the lumbar back to the sacrum.This unique anatomical structure enables the polyfidus muscle to produce strong contractile force,thus maintaining the stability of the spine.When the lumbar polyfidus muscle is injured and atrophied,the stability of the spine decreases accordingly,resulting in disturbance of facet joint activity,local muscle spasm,peripheral ligaments and disc injury,etc.,which can cause pain.Research Purpose and SignificanceAcupuncture therapy,as the representative of traditional Chinese therapy,has the functions of relaxing meridians and dredging collaterals,regulating qi and blood,relieving spasm and pain.In 1980,the World Health Organization listed low back pain as the dominant disease of acupuncture treatment.A large number of modern studies also show that acupuncture can effectively alleviate the affected area by eliminating inflammation and edema of tissues,improving local blood circulation and reducing tension of local muscle tissues.Symptoms such as pain and limited movement.The mechanism of acupuncture treatment has been constantly confirmed and updated by clinical workers at home and abroad,but up to now,there are few studies on the timing of acupuncture intervention,and the conclusions are different,but all of them suggest that the timing of acupuncture intervention affects the clinical efficacy to a certain extent.Previous studies have found that electro-acupuncture at Weizhong and Shenshu points can increase the expression of myogenic factors such as Pax-7 and IGF-1,reduce inflammation and accelerate the repair of injured polyfidus muscle.On the basis of this experiment,by observing the effects of different intervening time of Electroacupuncture on the expression of myodysplastic antigen Myod,cell cycle-related protein CDK4 and P57 of muscle satellite cells,we further explored the mechanism and optimal intervening time of electroacupuncture,and provided reliable evidence for standardizing the treatment of low back pain and improving the effectiveness of acupuncture.Research method42 SD rats were randomly divided into blank control group,model 1 group(immediate fixation),model 2 group(24 hours fixation),model 3 group(48 hours fixation),electro-acupuncture 1 group(immediate electro-acupuncture),electro-acupuncture 2 group(24 hours electro-acupuncture)and electro-acupuncture 3 group(48 hours electro-acupuncture),each group had 6 rats.The model of polyfidus muscle injury was established by intramuscular injection of 0.5%bupivacaine.The electro-acupuncture group was treated with bilateral "Weizhong" and"Shenshu"points once a day for 20 minutes,and the samples were taken one day after the acupuncture.The model group and the blank control group did not interfere,and sampled materials synchronously with the acupuncture group.Western Blot was used to detect the expression of Myod and P57,and immunohistochemistry was used to detect the positive expression of CDK4.Research findingsThe expression of Myod protein in EA group 1,2 and corresponding model group was higher than that in blank group(P<0.01),and the expression of Myod protein in EA group 2 was higher than that in model group 2(P<0.01);compared with EA group,the expression of Myod protein in EA group 1 and EA group 2 was significantly higher than that in EA group 3(P<0.01),and the expression of Myod protein in EA group 2 was higher than that in EA group 1(P<0.05).The expression of P57 protein in EA 2 group was lower than that in model 2 group(P<0.05),but there was no significant difference between other groups.The expression of CDK4 protein in EA group and model group was higher than that in blank group(P<0.01 or P<0.05),and the expression of CDK4 protein in EA group 1 and 2 was higher than that in corresponding model group(P<0.01 or P<0.05);compared with EA group,the expression of CDK4 protein in EA group 2 was higher than that in EA group 3(P<0.05).Research conclusionElectro-acupuncture can promote the benign repair of injured polyfidus muscle,and the best intervention effect is 24 hours.The mechanism may be related to the increase of Myod and CDK4 expression. |
