| ObjectiveExplore psoriasis vulgaris of modern anomalies and the quality of life between different syndromes have differences,as well as the effect in patients with psoriasis vulgaris anxiety,depression,degree of related factors.The historical origin and specific application of"all pains and itching sores belong to the heart" were sorted out to provide a preliminary basis for "treating psoriasis from the heart"and broaden the clinical application ideas.MethodsUsing cross-sectional survey research method,random collection between December2017 and March 2019 in Beijing Chinese medicine hospital dermatology clinic or ward,has been diagnosed with psoriasis vulgaris patients,by the deputy director of the physician and above syndrome after diagnosis belongs,collecting clinical information,USES the detected area and severity index(PASI score)assessment of patients with skin conditions;The psoriatic incompetence index(PDI)and dermatological quality of life index(DLQI)were used to evaluate the effect of psoriasis on life.Depression screening scale(PHQ-9)and GAD-7 were used to assess the status and severity of depression and anxiety.The relationship between the age,gender,educational background,family history of psoriasis,course of disease,age of first onset,onset and aggravating inducement,degree of pruritus and other factors and the degree of syndrome and emotional abnormality was analyzedResultsA total of 101 eligible cases were enrolled,aged from 8 to 79 years.Among them,66(65.3%)were male patients and 35(34.7%)were female patients.The PASI score ranged from 0.9 to 39.6,with an average(±standard deviation)of 10.70(±7.79).The number of people with anxiety and depression accounted for 76.24%of the total.1.Comparing the correlations of various factors with depression and anxiety state.Results:The PASI score representing the severity of psoriasis skin lesion and PHQ-9 representing the severity of depressive symptoms were analyzed by linear regression method,and P=0.628(>0.05);the GAD-7 score representing the severity of anxiety symptoms was compared with P=0.930(>0.05),and there was no significant difference;the pruritus of psoriasis was treated with the GAD-7 score representing the severity of anxiety symptoms.The degree of psoriasis itching was compared with the severity of anxiety,P=0.025(<0.05),the difference was statistically significant.Speman correlation analysis showed that the correlation coefficient B=0.207,P=0.038(<0.05).It can be considered that there is a positive correlation between them.The more obvious itching,the more serious depression and anxiety.According to whether there is head and face involvement,the whole group was divided into two groups,and compared with PHQ-9 score,P=0.390(>0.05),compared with GAD-7 score,P=0.337(>0.05).There was no significant difference in the degree of depression and anxiety between the two groups.According to the educational background distribution,the whole group was divided into two groups,and compared with PHQ-9 score,P = 0.390(>0.05),which was progressed with GAD-7 score.By comparison,P=0.337(>0.05),it was suggested that there was no significant difference in the severity of depression and anxiety among the groups grouped by educational background.The DLQI scale and PDI scale were compared with the scores of PHQ-9 and GAD-7 scales by linear regression method.By comparing the values of DLQI scale with those of PHQ-9 scale,0.021(<0.05),B=0.204 were obtained.The scores of DLQI scale and GAD-7 scale were improved.By comparison,P=0.004(<0.01),B=0.264;it can be seen that there is a significant positive correlation between the quality of life score and depression score of psoriasis patients,that is,the more the quality of life of patients declines,the heavier the degree of depression;by comparing the score of PDI scale with that of PHQ-9 scale,P=0.001(<0.01),B=0.172;by comparing the score of PDI scale with that of GAD-7 scale,P=0.001(<0.01),P=0.001(<0.172).The lower the PDI score,the higher the level of disability and the worse the ability to live.The results showed that the worse the ability to live,the worse the degree of depression and anxiety.2.To analyze the risk factors related to the onset and recurrence of psoriasis:comparing the heredity with PASI score,P=0.1(>0.05),indicating that there is no significant difference in the severity of psoriasis;comparing the age of the first onset with the heredity,P=0.731(>0.05),there is no significant difference in the age of the first onset,suggesting that there is no significant difference in the age of the first onset of psoriasis;Sexual regression analysis showed that the degree of pruritus and DLQI score were P= 0.000(<0.01),B = 3.251;compared with PDI score,P=0.008(<0.01),B=-4.818,both of which were significantly correlated,suggesting that the degree of pruritus was significantly correlated with the quality of life and disability index,and that the increase of the degree of pruritus would significantly reduce the quality of life of patients and seriously affect their ability to live.3.Relevant analysis of TCM syndromes:There were 56 cases of blood-heat syndrome,32 cases of blood-dryness syndrome and 13 cases of blood-stasis syndrome in 101 patients,with an average age of 38.01±16.66 years;the course of disease ranged from 1 year to 57 years,with an average duration of 12.57±11.90 years.Comparing different syndrome types with PHQ-9 score,P=0.910(>0.05);comparing different syndrome types with GAD-7 score,P=0.510(>0.05),there was no significant difference in anxiety and depression between different syndrome types;DLQI score of blood-heat syndrome was 8.84(±7.17);PDI score was 61.07(±13.34);DLQI score of blood-dryness syndrome was6.68(±4.39);PDI evaluation.The scores of DLQI and PDI of blood stasis syndrome were 65.06(±7.88),9.31(±6.07)and 66.15(±6.56),respectively.Compared with DLQI score,P=0.505(>0.05)and P=0.663(>0.05)showed no significant difference between different syndromes,suggesting that there was no significant difference in DLQI and PDI score between different syndromes groups;after comparing the course of disease,age of initial onset,itching degree of skin,PASI score,P=0.011(<0.05)in the age of initial onset.It suggests that the age of onset is different among different syndromes.After two comparisons,it is concluded that the initial onset age of patients with blood stasis syndrome is significantly older than that of patients with blood dryness syndrome,and there is no significant difference between the other two comparisons;in the course of disease,P=0.045(<0.05),suggesting that the duration of disease is different between different syndrome types.After two comparisons,the duration of patients with blood dryness type is significantly longer than that of the other two types;in PASI score,P=0.605(>0.05),skin.The degree of pruritus P=0.550(>0.05),suggesting that there was no significant difference in PASI score and skin pruritus among patients with different syndromes.ConclusionsPsoriasis is a psychosomatic disease,and its itching symptoms are closely related to the patients’ own mental mood.The degree of itching and the degree of life influence are related to the degree of depression and anxiety.There was no significant difference in the tendency of anxiety and depression symptoms between different psoriasis syndromes,and depression symptoms were often parallel with anxiety symptoms,but the degree of anxiety symptoms was deeper in patients with severe anxiety than in patients with severe depression. |
