Objetive:Traditional lumbar fusion is widely recognized as a standard surgical method for lumbar disc herniation,lumbar pain and neurologic dysfunction,However,it has many disadvantages such as large trauma,high stability of posterior lumbar spine and high cost,As a new technique of lumbar fusion in recent years,it is widely used in clinical practice.The purpose of this study was to investigate the clinical efficacy of intervertebral fusion for lumbar disc herniation.methods:The 40 cases of lumbar intervertebral disc herniation were treated by using the open intervertebral fusion device between January 2015 and January 2017 in the spinal surgery ward of affiliated sports hospital of chengdu sports university,Among them,18 females and 22 males were analyzed systematically.Collect patients with preoperative and postoperative and postoperative 3 months,6 months after different time points of ODI(Oswestry disability index questionnaire)and VAS(visual analogue scale method)score,general situation of operation,postoperative imaging were analyzed,and the different time to assess can open type of intervertebral fusion in the treatment of lumbar intervertebral disc protrusion clinical curative effect is reliable.results:All patients by the same surgeon successfully completed surgery,surgery time average 74 min,surgical blood loss,on average,137 ml,average 7 cm incision,intraoperative fluoroscopy,on average,6 times,postoperative drainage average 35 ml,average down time after 18 days,the average hospitalization time of 22 days.Preoperative ODI average 36.7 points,postoperative ODI averaged 32.0 points,after march DOI averaged 24.5 points,after June ODI averaged 17.1 points,the last follow-up ODI averaged 12.9 points,before and at the end of the follow-up period ODI score was 65%.Preoperative VAS 7.2,postoperative VAS average 5.7,VAS averaged 4.4 points after surgery,VAS averaged 3.1 points in June,and VAS averaged 2.3 points in the final follow-up,and the improvement rate of VAS score was 67% before and after surgery.Preoperative X-ray power average relative Angle is 3.4°,1 years after X-ray power an average relative Angle is 2.5°preoperative power a relative displacement of the vertebral bodies with an average of 2.0 mm,1 years after power a relative displacement of the vertebral bodies with an average of 1.4 mm.Preoperative intervertebral disc height average 10.6mm,postoperative intervertebral disc height average 13.3mm,postoperative intervertebral disc height,on average,12.8mm in March,postoperative intervertebral disc height,on average,12.6mm in June,the last follow-up of intervertebral disc height average 12.4mm.The height of intervertebral clearance was 25.3%,and the interval height was 45%,and the interval height of the final follow-up was 43%.All the 40 patients were in a balanced condition before the operation,and all 40 patients in the last follow-up were in equilibrium,and the balance of the coronal surface was maintained at 100%.At the end of the follow-up,3 of all 40 patients were in unconfluence,and the intervertebral space fusion rate was 92.5%.Conclusion:It can effectively treat lumbar intervertebral disc herniation,restore the biomechanics of the spine,restore and maintain the height of intervertebral space,and can effectively integrate the lesion segments. |
