Background and Objective New coronavirus pneumonia(coronavirus disease2019,COVID-19)is an acute infectious pneumonia caused by severe acute respiratory syndrome coronavirus 2(SARS-Co V-2),which broke out in China at the end of 2019 and swept the world rapidly.Its main clinical symptoms are fever,cough and fatigue.advanced age,combined with hypertension,coronary heart disease,heart failure and other cardiovascular diseases are high risk factors for COVID-19 disease and poor prognosis.Hypertension is a common disease in the world.It is mainly manifested in arterial pressure in systemic circulation,including clinical syndrome with persistent elevated systolic and / or diastolic blood pressure.The etiology is unclear and there may be many causes and inducements.Referring to the Chinese guidelines for the Prevention and treatment of Hypertension(2018 revised edition),for the definition and diagnostic criteria of "hypertension ",hypertension is defined as: in the absence of antihypertensive drugs,Three indirect blood pressure measurements in non-homogeneous room showed systolic blood pressure ≥140 mm Hg and / or diastolic blood pressure ≥90 mm Hg,.Because the etiology is not completely clear,hypertension can be divided into essential hypertension and secondary hypertension.Essential hypertension refers to a clinical syndrome in which the etiology is unknown and the interaction of multiple factors,especially genetic and environmental factors,but there is no complete and unified understanding of the specific ways to lead to elevated blood pressure.The latter is usually associated with other causes of hypertension,including vascular diseases,endocrine system diseases,etc.Hypertension usually coexists with other cardiovascular disease risk factors and can not be ignored.Recently,the increase of epicardial adipose tissue(EAT)may be an important factor affecting the occurrence of many cardiovascular diseases and metabolic syndrome.A large number of studies have confirmed that the thickness and volume of EAT are markers of early risk of cardiovascular disease.CT imaging technology has become the most important clinical method for quantitative evaluation of epicardial fat in recent years because of its high accuracy,high safety and simplicity.This paper aims to explore the laboratory and CT imaging characteristics of myocardial injury in patients with new coronavirus infection pneumonia(COVID-19)complicated with hypertension.Methods A retrospective analysis of COVID-19 patients admitted to Anqing Municipal Hospital from January 22 to March 4,2020,It was divided into two groups according to whether it was complicated with hypertension.To compare the indicators of laboratory examination between the two groups,The CT value of myocardium and the volume of epicardial fat(EAT)were obtained by computer tomography(CT),Analysis COVID-19 myocardial imaging characteristics of patients with hypertension.Results Comparing the general baseline data between the two groups,patients had no significant difference in gender,age,composition of underlying diseases(P>0.05).There was no significant difference in baseline laboratory tests including hs-TNT between the two groups(P>0.05),The peak hs-TNT of COVID-19 patients with hypertension was significantly higher [4.00(3.00)] than those without hypertension,6.00)vs.9.50(6.95),10.45),P =0.003].To compare CT imaging characteristics of the two groups,we can find that there was no significant difference in myocardial CT between baseline and pre-discharge patients(P>0.05).There was no significant difference in baseline EAT volume between the two groups(P=0.176),The volume of EAT in hypertension group was significantly higher than that in non-hypertension group [(180.09±33.13)vs.(114.63±42.30),P<0.001],The volume difference between the two groups EAT admission showed a significant increase in EAT volume in hypertension group [(59.89±24.62)vs.(13.43±21.18),P=0.004].Conclusion The peak hs-TNT of COVID-19 patients with hypertension and the CT volume measured by hospital CT were significantly higher than those without hypertension,suggesting that the disease may have potential myocardial injury in patients with hypertension. |
