Objective1.To investigated the status quo of social support,perceived stress,self-management behavior and quality of life in patients with rheumatoid arthritis;2.Identify the correlation between social support,perceived stress,self-management behavior and quality of life in patients with rheumatoid arthritis;3.Explore the influencing factors of social support,perceived pressure,self-management behavior,and quality of life;4.Exploring the mediating effects of perceptual pressure and self-management behaviors in patients with rheumatoid arthritis on social support and self-management behaviors and the path between the four.MethodsThe convenience sampling method was used to select patients with rheumatoid arthritis who were treated in the rheumatology and immunology department of a tertiary Class A hospital in Tianjin as the research object.The general data questionnaire,social support assessment scale,Chinese version of the perceived stress scale,and chronic disease self The Management Behavior Scale and the Rheumatoid Arthritis Patients’ Quality of Life Scale(the higher the score,the worse the quality of life)was performed on 482 rheumatoid arthritis patients who met the inclusion and exclusion criteria from May to September 2019.Questionnaire.Use the mean,standard deviation,composition comparison to describe the general information of the study subjects,social support,perceived pressure,self-management behavior and quality of life;use single-factor analysis of variance,two independent samples t test to analyze the life of patients with rheumatoid arthritis Demographic differences in quality;use multivariate stepwise regression to analyze factors affecting quality of life;use Pearson analysis to explore the relationship between patients’ social support,perceived pressure,self-management behavior and quality of life;use mediating effect method to explore Intermediate role between social support and self-management behavior and establish structural equation model.Results1.The average score of total social support for patients with rheumatoid arthritis is(34.46±4.52),which is at a medium level;different ages,marital status,living status,payment methods,working status,course of disease,degree of joint deformation,degree of pain,Patients with rheumatoid arthritis with graded joint function had statistically significant differences in social support scores(P<0.05).2.The total score of perceptual stress is(33.91±6.61),which is at a high level;different gender,age,marital status,place of residence,living status,education level,payment method,working status,average monthly family income,course of illness,Rheumatoid arthritis patients with joint deformation degree,pain degree,joint function grading have statistically significant differences in the perceptual pressure score(P<0.05).3.The average score of self-management behavior is(23.24±6.01),which is at a low level;different ages,marital status,place of residence,living conditions,education,payment methods,joint deformation,pain,joint function classification of patients with rheumatoid arthritis had statistically significant differences in self-management behavior scores(P<0.05).4.The average score of quality of life is(75.22±11.09),which is at a poor level;different ages,marital status,place of residence,living status,education level,payment method of diagnosis and treatment,working status,average monthly family income,course of illness,Rheumatoid arthritis patients with joint deformation degree,pain and joint function grades had statistically significant differences in quality of life scores(P<0.05).5.Correlation analysis results show that the score of rheumatoid arthritis social support is negatively correlated with perceptual stress score and quality of life score(quality of life scale is inverse vector scale),and positively correlated with self-management behavior score;perceptual stress score is related to quality of life The score was positively correlated and negatively correlated with self-management behavior scores;the self-management behavior score was negatively correlated with quality of life scores.6.The results of multiple stepwise regression analysis showed that: marital status,joint deformation,and living status jointly explain the 20.7% variability of social support in patients with rheumatoid arthritis;total social support score,joint deformation,marital status,The eight variables of gender,joint function grade,age,monthly household income per capita,and living status can jointly explain the 27.7%variability of perceptual pressure in patients with rheumatoid arthritis;8 variables including education level,age,pain level,and working status can jointly explain the31.2% variability of self-management behavior in patients with rheumatoid arthritis;This variable can explain 55.2% of the total variation in the quality of life of rheumatoid arthritis patients.7.The structural equation model shows that: Perceived pressure plays a partial intermediary role in the path of social support and quality of life,and the intermediary effect value accounts for 33.74% of the total effect;self-management behavior plays a partial intermediary role in the path of social support and quality of life.The intermediary effect value accounts for 25.29% of the total effect;perceptual stress and self-management behavior play a part of the intermediary role in the path of social support and quality of life,and the intermediary effect value accounts for 50.66% of the total effect.Conclusion1.Social support for rheumatoid arthritis patients is at a medium level;perceptual stress is at a high level;self-management behavior is at a low level;quality of life is at a poor level.2.The social support score is negatively correlated with the perceptual stress score and quality of life score,and positively correlated with the self-management behavior score,that is,the better the social support,the lower the perceived pressure,the better the quality of life;the perceived stress score and the self-management line score are There is a negative correlation,which is positively correlated with the quality of life score,that is,the higher the perceived pressure,the worse the self-management behavior and quality of life;the self-management behavior score is negatively correlated with the quality of life score,that is,the better theself-management behavior,the better the quality of life.3.Demographic data and disease-related data have a certain impact on the social support,perceived stress self-management behavior and quality of life of rheumatoid arthritis patients.Clinical medical personnel should implement individualized and targeted health education and nursing measures according to different factors.4.Social support can directly affect the quality of life of rheumatoid arthritis patients,and can also indirectly affect the quality of life through perceptual stress and self-management behaviors.
