Objective:To establish a new technique and method for detecting carbohydrate-deficient transferrin,an indicator of alcohol dependence,and analyze the differences of content of carbohydrate-deficient transferrin(CDT)in three groups as non-drinkers,occasional drinkers and alcohol abusers in Xinjiang.Carbohydrate-deficient Transferrin(CDT)is used as an efficient alcohol biomarker to provide scientific detecting data and references for the clinical diagnosis and therapeutic assessment of alcohol abuse and forensic identification in DUI.Methods:The CDT content of serum were measured in three groups as non-drinkers,occasional drinkers and alcohol abusers in Xinjiang with the Capillarys electrophoresis and Capillarys CDT kit.The capillary electrophoresis and Capillarys CDT kit were used to detect CDT as the gold standard method.The results of HPLC-UV analysis were compared with those of gold standard to evaluate the effect of HPLC-UV on CDT effective performance.The quality control samples and the blank samples control method were used to measure the content of carbohydrate-deficient transferrin(CDT).The contents of alanine aminotransferase(ALT),aspartate aminotransferase(AST),glutamase transferase(GGT)and mean corpuscular volume(MCV)in drinkers were detected by automatic biochemical analyzer.Then,using statistical software compare CDT with other alcohol biochemical indicators AST,ALT,GGT and MCV.Results:High performance liquid chromatography(HPLC)has good sensitivity95.7%and specificity 92.3%,and the gold standard method of capillary electrophoresis(CZE)has a good linear relationship,the regression equation was Y=0.8968x+0.691 3,R~2=0.935 1.Mean±SD of CDT in non-drinkers healthy group(N=73)was0.699±0.202,Mean±SD of CDT in occasional drinkers group(N=154)was 0.964±0.281,Mean±SD of CDT in alcohol abusers group(N=141)was 5.826±1.793.Compared with the non-alcoholic and occasional drinkers,CDT contents in alcohol-dependent group were significantly increased,the difference was statistically significant(P<0.001).The MCV positive rate in occasionally drinkers group was 54.5%,the GGT positive rate was 37.0%,the CDT positive rate was5.2%.The MCV positive rate in the alcohol-dependent group was 75.2%,the GGT positive rate was 79.4%,the GGT positive rate was 99.3%.Conclusion:Compared to the routine clinical biomarkers of ALT and AST,CDT is the most sensitive and specific marker,it will serve as a marker of judicial identification.
