Objective: Anterior cruciate ligament reconstruction is often accompanied by disuse-atrophy of quadriceps femoris, but there is a lack of a simple and efficient method to evaluate the rehabilitation of quadriceps femoris after operation. The purpose of this study is to evaluate the muscle strength of quadriceps femoris by measuring the elastic modulus of quadriceps femoris after operation through shear wave elastography,so as to guide the rehabilitation treatment and improve the effect of operation. Methods: A total of 42 patients who underwent unilateral anterior cruciate ligament reconstruction in the affiliated Hospital of Guizhou Medical University from January 2018 to January 2019 were selected.one year of knee rehabilitation training was completed after operation. The thickness of bilateral vastus lateralis, rectus femoris and medial vastus was measured by conventional two-dimensional ultrasound. The elastic moduli of bilateral vastus lateralis, rectus femoris and medial vastus were measured by shear wave elastography, and the thickness difference between the affected side and the healthy side, the elastic modulus difference in the relaxed state and the elastic modulus difference in the maximum contraction state were recorded. All patients completed Lysholm score and IKDC score. To explore the difference of muscle thickness and elastic modulus of quadriceps femoris in patients with different knee joint function scores. Results: 1.There was no significant difference in the average thickness difference of vastus lateralis, rectus femoris and medial vastus in patients with different knee joint function(P > 0.05).There was a significant difference in the average elastic modulus difference between the relaxed state and the maximum contraction state(P < 0.05).2.After operation, there were significant differences between medial vastus muscle and vastus lateralis muscle in muscle thickness difference, elastic modulus difference in relaxed state and elastic modulus differencein maximum contraction state(P < 0.05).The counting data of medial vastus muscle(muscle thickness difference, elastic modulus difference in relaxed state, elastic modulus difference in maximum contraction state)had higher correlation with knee joint function score. Conclusion:Measuring the elastic modulus of vastus lateralis, rectus femoris and medial vastus by shear wave elastography can not only evaluate the recovery of muscle thickness and muscle strength, but also quantify the imbalance of muscle strength of the affected knee joint. It provides a new idea for guiding rehabilitation treatment after anterior cruciate ligament reconstruction.
