A Study On Group Work Intervention Self-management Of Elderly Chronic Patients

With the accelerated pace of aging,the number of people suffering from chronic non communicable diseases is increasing.These elderly people not only have to endure the pain of suffering,but also bear enormous psychological pressure,which leads to their poor health management consciousness.Advocating the concept of health management for elderly chronic patients,stimulating their self-management awareness and improving their health management ability.It is an important subject that scholars need to study at present.Based on the field research,based on the theory of empowerment and social support,this study is based on the questionnaire survey and structured interview of elderly patients with chronic diseases.Elderly patients with chronic diseases have problems such as weak self-management consciousness,passive medical treatment,traditional psychological pressure,and poor social support system.Based on these problems,group activities are carried out to help elderly patients with chronic diseases master the correct management methods,overcome negative emotions,and develop healthy lifestyles and eating habits.It aims to improve the self-management ability of the elderly patients with chronic diseases.Self-management of elderly chronic patients is a comprehensive project of community work.Through the practice of social workers’ self-management,this paper theoretically enriches the content of self-management research on elderly chronic patients in China.In practice,this paper explores the ways and adaptability of using the social work group service method to promote the self-health management of elderly chronic patients.In order to solve the problems existing in the management of elderly chronic patients,the following Suggestions are put forward: first,give play to the advantages of social workers in health management,and use the knowledge learned to help elderly patients relieve stress;Second,social workers should actively promote health management so that more elderly patients with chronic diseases can understand the connotation of health management.Third,contact multiple departments to link resources;Fourthly,it advocates to build a new model of health management involving multiple parties.Due to the limited number of selected communities,it is not possible to compare different communities,such as rural communities and urban communities,open communities and gated communities,in terms of self-management methods,awareness and abilities of elderly chronic patients.Therefore,the thesis still has the very big deficiency.In the future,different community types should be included in the framework of analysis,so as to further promote the development of community elderly chronic patients’ self-management and social work intervention mechanism.
